Duolin Respules (ipratropium bromide + salbutamol) can be mixed with normal saline (0.9% NaCl) for nebulization — but not with plain tap water or sterile water.
Here's what you need to know:

Can Duolin Neb be diluted?

Yes — with normal saline only.
Duolin Respules contain ipratropium bromide + salbutamol (albuterol). Standard nebulization protocol dilutes the solution with 3 mL of normal saline (0.9% NaCl) when using a concentrated solution, to achieve an adequate fill volume for the nebulizer cup (typically 3–5 mL total).
  • If you're using a unit-dose respule (pre-filled, ready-to-use), it may already contain enough volume — check the label. Many unit-dose respules can be used directly without further dilution.
  • If dilution is needed, use sterile normal saline (0.9% NaCl) — not tap water, distilled water, or sterile water for injection.

Why not plain water?

Plain water (hypotonic) can cause bronchospasm when nebulized, because it creates an osmotic mismatch with airway mucosa. Normal saline is isotonic and safe for the airways.
