TVP and LVP arrangement on the Eustachian tube
This illustration (by Gralapp, from the Stanford Oto Surgery Atlas) shows exactly the arrangement tested in INICET May 2026:
Key anatomical points shown in the image:
  • LVP (Levator Veli Palatini) - runs as a broad band on the posteromedial/inferior side of the Eustachian tube cartilage. It elevates the floor of the ET and moves the medial lamina toward the midline. The black arrows show how it compresses/opens the tube lumen.
  • TVP (Tensor Veli Palatini) - lies on the anterolateral side, running inferiorly before hooking around the pterygoid hamulus. Its medial bundle (dilator tubae) is the PRIMARY dilator of the Eustachian tube.
  • The red structure on the right is the internal carotid artery, a key surgical landmark.
Summary for exam:
FeatureTVPLVP
Position relative to ETAnterolateralPosteromedial/inferior
InnervationV3 (mandibular n.)CN X (vagus) via pharyngeal plexus
Primary roleOpens/dilates ET (via dilator tubae)Elevates palate; assists ET opening
Hook aroundPterygoid hamulus-
TVP is the main muscle responsible for ET opening and middle ear ventilation.
