Clinical Impression: This ECG is Highly Concerning for Acute Anterior (Extensive) STEMI — Treat as a Cardiac Emergency
Reading the tracing:
Rate ~99 bpm, sinus rhythm (rhythm strip II).
Paper: 25 mm/sec, 10 mm/mV — standard calibration, so the amplitudes are true, not an artifact.
V1–V6 show large, broad, "tombstone"-shaped monophasic upward deflections with marked ST-segment elevation, most pronounced in V2–V4 and extending into V5/V6. This pattern (wide, coved, tombstoning ST elevation across the precordial leads) is classic for an acute, extensive anterior wall STEMI, most often due to a proximal/mid LAD occlusion. Tombstone morphology specifically correlates with larger infarct size and higher risk.
Limb leads (I, II, III, aVL, aVF) show comparatively small QRS complexes, and aVR is negative as expected — no evidence pointing to a different rhythm disturbance on this strip.
This corresponds exactly to what a textbook describes: "ST segment elevation is seen in leads V1 to V4 (anterior leads) and in leads I, aVL, V5, and V6 (lateral leads)" for anterior/anterolateral STEMI - Rosen's Emergency Medicine, Concepts and Clinical Practice.
Bottom line: A 50-year-old woman with 3 hours of chest pain and this ECG must be managed as a STEMI until proven otherwise — this is not a "wait and see" ECG.
What needs to happen immediately (do not delay for troponin)
Activate the cath lab / STEMI protocol now. Time is myocardium — goal door-to-balloon time is 90 minutes (or fibrinolysis within 30 minutes if primary PCI isn't available within 120 minutes).
P2Y12 inhibitor (ticagrelor or prasugrel preferred, clopidogrel if on anticoagulant or high bleeding risk) as per current ACC/AHA and NICE STEMI pathways.
Anticoagulation (unfractionated heparin, or per local primary-PCI protocol).
Oxygen only if SpO2 <90% — routine oxygen is no longer recommended in normoxic patients.
Pain control: IV opioid (morphine) cautiously — note the CIRCUS study flagged morphine can delay antiplatelet absorption in anterior STEMI, so this is adjunctive, not primary treatment. Nitrates can help if BP tolerates it (avoid if hypotensive, right ventricular infarct suspected, or recent PDE5-inhibitor use).
Beta-blocker early only if hemodynamically stable, no signs of heart failure/cardiogenic shock, and no contraindication.
Get a stat BP and full vital signs — the vitals field on this printout is unclear/illegible, and BP critically affects whether nitrates/morphine can be given safely, and whether the patient is already in cardiogenic shock (large anterior STEMIs carry the highest risk of this).
Continuous cardiac monitoring for arrhythmia (VF/VT risk is highest in the first hours), IV access, serial ECGs, and troponin sent (but treatment should not wait for the result).
This aligns with the 2025 ACC/AHA/ACEP/NAEMSP/SCAI ACS guideline (PMID: 40014670) and NICE NG185, both of which emphasize immediate reperfusion without waiting for biomarker confirmation once STEMI is identified on ECG.
Recommendation: If this patient is in front of you or a colleague right now, get her on a monitor, give aspirin, and call for emergency transfer/activation to a PCI-capable facility immediately — please treat this as time-critical rather than waiting for further discussion.
