short PR interval ECG interpretation pre-excitation authoritative guideline

This ECG appears largely normal, with:
  • Sinus rhythm at about 80 beats/min
  • Normal QRS duration: 76 ms
  • QTc about 429 ms, which is generally within normal range
  • No obvious acute ST elevation/depression or major T-wave abnormality visible in this photograph
  • The automated report notes a short PR interval: about 104 ms (normal is usually 120-200 ms)
A short PR can be a normal variant, but it sometimes reflects faster-than-usual AV-node conduction or, less commonly, an accessory electrical pathway. In classic WPW pre-excitation, there is also usually a wide QRS and delta wave; those are not obvious here. This is consistent with the machine wording: “Sinus rhythm with short PR, otherwise normal ECG.” A short PR by itself does not establish WPW. MSD description of WPW ECG features
So: not completely “normal” because the PR is short, but there is no obvious dangerous acute abnormality on this photo. The tracing should be reviewed by the clinician who ordered it, particularly if you have palpitations, fainting, chest pain, breathlessness, or a family history of sudden cardiac death.
Seek urgent care now for chest pressure/pain, fainting, severe shortness of breath, or a sustained fast/irregular heartbeat.
