APACHE II Score

APACHE II = Acute Physiology Score (APS) + Age points + Chronic health points
It is an ICU severity-of-illness score calculated from the worst values during the first 24 hours of ICU admission. Total score: 0-71. Higher score indicates greater severity and mortality risk.

1. Acute Physiology Score: 12 variables

Each variable is allotted 0-4 points according to deviation from normal.

  2. Mean arterial pressure
  3. Heart rate
  4. Respiratory rate
  5. Respiratory oxygenation
    • FiO₂ <50%: use PaO₂
    • FiO₂ ≥50%: use A-aDO₂ gradient
  6. pH arterial
    • If ABG unavailable, use serum HCO₃⁻
  7. Serum sodium
  8. K serum potassium
  9. Creatinine
  10. Hematocrit
  11. WBC count
  12. Glasgow Coma Scale: 15 - actual GCS
Note: Double the creatinine score in acute renal failure.

2. Age points

3. Chronic health points

Given in severe chronic organ insufficiency or immunocompromised state:
  • Non-operative or emergency postoperative patient: +5
  • Elective postoperative patient: +2

Uses

  • Assesses severity of illness in ICU patients
  • Predicts mortality and permits comparison of ICU outcomes
  • Helps in prognosis and audit, but should not be the sole basis for individual treatment decisions
