For Parkinson disease, levodopa is usually prescribed with carbidopa. Dosing is individualized by the prescriber.
  • Immediate-release carbidopa/levodopa (common starting approach): 25/100 mg (25 mg carbidopa + 100 mg levodopa) three times daily, then adjusted gradually based on benefit and adverse effects.
  • Another labeled starting regimen is one 25/100 mg tablet three or four times daily.
  • Aim for at least 70-100 mg/day of carbidopa to limit nausea from peripheral levodopa conversion.
  • The dose may later be divided more frequently, often every 3-4 hours, if wearing-off occurs. Use the lowest dose that gives satisfactory benefit, since higher doses can worsen involuntary movements (dyskinesia).
  • Extended-release products are not interchangeable milligram-for-milligram with immediate-release products. For example, CREXONT has a labeled starting dose in levodopa-naive adults of 35/140 mg twice daily for 3 days, then titrated.
Do not stop levodopa abruptly, because this can cause a dangerous severe withdrawal syndrome with rigidity, fever, and confusion. Take it exactly as prescribed and contact the prescriber for worsening movements, hallucinations, fainting, severe nausea, or sudden sleep episodes.
