Brief overview: NSAIDs work by inhibiting cyclooxygenase (COX-1 and/or COX-2) enzymes, blocking prostaglandin synthesis - this reduces pain, fever, and inflammation but also removes the protective prostaglandin effects on gastric mucosa and renal blood flow, which is the root of their two major toxicities (GI ulceration/bleeding and renal impairment). COX-2 selective agents were developed to reduce GI risk but carry increased cardiovascular risk instead. They are among the most widely used drug classes worldwide but require careful risk-benefit assessment, especially in the elderly, in renal/cardiac disease, and with concurrent anticoagulation.
