Syp Sucrafil-O (Sucralfate) - Timing of Administration
Give BEFORE food - on an empty stomach.
Correct Timing:
1 hour before meals (breakfast, lunch, dinner)
At bedtime (as a 4th dose, on empty stomach)
Avoid eating for at least 1 hour before and 2 hours after the dose
Why It Must Be Given Before Food (Mechanism):
Sucralfate works locally in the stomach - it does NOT get absorbed into the bloodstream. In an acidic, empty stomach environment:
It reacts with hydrochloric acid (HCl) to form a thick, viscous, paste-like substance
This paste binds selectively to the ulcer/erosion site (has high affinity for damaged mucosa)
It forms a physical protective barrier over the ulcer for up to 6 hours
It also stimulates mucus and bicarbonate secretion and promotes healing
If given after food, food buffers the stomach acid, reducing the activation of sucralfate and its ability to bind to the ulcer site - significantly reducing its effectiveness.
Standard Dosing Schedule:
Time

30-60 min before breakfast
10 mL (1g)
30-60 min before lunch
10 mL (1g)
30-60 min before dinner
10 mL (1g)
At bedtime
10 mL (1g)
Important Precautions:
Do NOT take antacids within 30 minutes of sucralfate (antacids raise pH and reduce its activation)
Do NOT take other medications simultaneously - sucralfate can bind to drugs like ciprofloxacin, digoxin, phenytoin, and reduce their absorption. Give other drugs 2 hours before sucralfate
Shake the suspension well before use
Sucrafil-O Specifically:
Sucrafil-O contains sucralfate + oxetacaine (oxethazaine) - a local anesthetic that also helps with pain/burning. The same "before food on empty stomach" rule applies to this combination.
In summary: always give 30-60 minutes before meals and at bedtime, never after food.
