What do you mean by 99th percentile

The 99th percentile in this context refers to a statistical cutoff value for troponin levels in a reference population of healthy individuals.

What it means:

  • Take 1,000 healthy people with no heart disease and measure their troponin levels
  • Rank all values from lowest to highest
  • The 99th percentile = the value at which 99% of healthy people fall below it
  • In other words, only 1% of normal, healthy people would have a troponin above this number

Why it matters in MI diagnosis:

Acute MI = Rise and/or fall of cTn with ≥1 value >99th percentile URL + evidence of ischemia
  • If a patient's troponin exceeds this threshold, their level is abnormal - higher than what you'd expect in 99% of healthy people
