  • Look: There's blood pooling around his lips and inside his mouth. You don't see a foreign body, but his mouth is bloody and he hasn't cleared it himself. His chest is moving, but you notice some see-saw type effort.
  • Listen: You hear that gurgling sound clearly when you put your ear close to his mouth - a wet, bubbling noise on each breath.
  • Feel: You feel warm, moist air on your cheek with each breath, so air is moving, just noisily.
Interpretation checkpoint: Gurgling = liquid (blood, in this case) sitting in the upper airway, partially obstructing it. He's not talking or protecting his own airway (he only groaned to voice, didn't open eyes or speak words - so his conscious level is reduced too, which is why he isn't clearing the blood himself).

On "removing the blood" - one refinement: since this is liquid blood, not a solid foreign object, the correct technique is suction (rigid or flexible catheter), not a finger sweep. A finger sweep is reserved for a visible solid foreign body, and even then you never do a blind sweep - pushing your finger in without seeing the object risks shoving it deeper. For liquid/secretions, suction is the tool.

On the OPA decision: Good instinct to reach for an airway adjunct given his reduced consciousness. One important technique point though - he's currently "P" on AVPU (responds to pain by groaning, no eyes, no words). That's borderline: OPA is meant for a patient with no gag reflex. So the way you actually find out is by trying it:
  • Insert the OPA (sized from corner of mouth to angle of jaw/earlobe).
  • If he tolerates it without gagging or coughing - confirms he's deeply unconscious enough, leave it in.
  • If he gags, retches, or tries to bite down - remove it immediately (risk of vomiting/aspiration) and consider a nasopharyngeal airway instead, since you correctly noted there's no facial/basilar skull trauma making NPA unsafe.
