Upper Respiratory Tract Infections (URTI)

Common Cold (Viral Rhinitis)

2. CARDIOVASCULAR SYSTEM

Hypertension (HTN)

Diagnosis: Confirmed BP ≥ 140/90 mmHg on two separate occasions (or 130/80 for high-risk)
Non-Pharmacological (ALL patients):
  • DASH diet (low sodium <5g/day), weight loss, aerobic exercise 150 min/week
  • Limit alcohol, stop smoking
Pharmacological (Stage 1: 140-159/90-99):

Irritable Bowel Syndrome (IBS)

4. ENDOCRINE SYSTEM

Type 2 Diabetes Mellitus (T2DM)

Targets: HbA1c <7% (most); FPG 80-130 mg/dL; PPG <180 mg/dL; BP <130/80; LDL <70 mg/dL
Step 1 - Lifestyle + Metformin:
  • Metformin 500 mg OD with food, titrate to 1000 mg BID (max 2-3 g/day)
  • eGFR guidance: reduce dose if eGFR 30-45; stop if <30
Step 2 - Add second agent based on comorbidities:
  • CVD/HF: SGLT-2i (empagliflozin 10 mg OD or dapagliflozin 10 mg OD) - reduce CV events and mortality
  • CKD: SGLT-2i (renoprotective); or GLP-1 RA (semaglutide 0.5-1 mg weekly SC)

Pharmacological:
  • Oral: Paracetamol 500-1000 mg TID (first line, mild-moderate)
  • NSAIDs: Ibuprofen 400-600 mg TID, Naproxen 250-500 mg BID, Diclofenac 75 mg BID (with PPI)
  • Topical NSAIDs: Diclofenac gel (preferred in elderly to minimize systemic effects)
  • Tramadol (if NSAIDs contraindicated): 50-100 mg TID
  • Intra-articular: Corticosteroid injections (methylprednisolone) - short-term relief

6. NEUROLOGICAL SYSTEM

Tension Headache

  • Paracetamol 1g PRN or Ibuprofen 400 mg PRN

  • Avoid analgesic overuse (>15 days/month - leads to medication overuse headache)

Migraine

Peripheral Neuropathy (Diabetic)

  • Glycaemic control: cornerstone
  • Pain (neuropathic):
    • Duloxetine 60 mg OD (FDA-approved, first line)
    • Pregabalin 75-150 mg BID
    • Gabapentin 300-1200 mg TID
    • Amitriptyline 25-75 mg nocte (alternative)

7. DERMATOLOGY

Atopic Dermatitis (Eczema)

8. URINARY SYSTEM

Urinary Tract Infection (UTI)

Uncomplicated Cystitis (women):
  • Nitrofurantoin 100 mg MR BID x 5 days (first line)
  • Trimethoprim 200 mg BID x 7 days
  • Fosfomycin 3g oral single dose
  • Cefalexin 500 mg TID x 5 days
Complicated UTI / Men / Pyelonephritis:
  • Ciprofloxacin 500 mg BID x 7-14 days; or Co-trimoxazole x 14 days
  • Culture-guided therapy preferred
Recurrent UTI (≥3/year):
  • Long-term low-dose prophylaxis: Nitrofurantoin 50 mg OD nocte
  • Post-coital prophylaxis: single dose after intercourse
  • Topical vaginal estrogen (postmenopausal women)

Benign Prostatic Hyperplasia (BPH)

Mild symptoms (IPSS <8): watchful waiting + lifestyle Moderate-Severe:
  • Alpha-blockers (symptom relief): Tamsulosin 0.4 mg OD or Alfuzosin 10 mg OD (first line)
  • 5-Alpha reductase inhibitors (shrink prostate): Finasteride 5 mg OD or Dutasteride 0.5 mg OD (prostate >40g)

12. WOMEN'S HEALTH

Dysmenorrhea (Primary)

  • NSAIDs (start 1-2 days before menses): Mefenamic acid 500 mg TID, Ibuprofen 400 mg TID
  • Combined OCP (reduces prostaglandin synthesis)
  • Heat therapy (evidence-based as NSAIDs for mild)
  • Tranexamic acid (if heavy flow)

Polycystic Ovary Syndrome (PCOS)

  • Lifestyle: weight loss (even 5-10% improves symptoms significantly)
  • Menstrual irregularity: Combined OCP (cyproterone acetate + ethinyl estradiol preferred for anti-androgenic effect)
  • Hyperandrogenism (hirsutism): Spironolactone 50-100 mg OD
  • Infertility: Clomiphene citrate (ovulation induction); Letrozole (preferred)
  • Insulin resistance/Metabolic: Metformin 500-1500 mg OD
  • Inositol supplementation (myo-inositol)

Menopause - HRT (Hormone Replacement Therapy)

  • Vasomotor symptoms: HRT (estrogen ± progesterone) - most effective
  • Women with uterus: Combined HRT (estrogen + progestogen)
  • Women post-hysterectomy: Estrogen alone
  • Non-hormonal (if HRT contraindicated): Venlafaxine 37.5-75 mg OD; gabapentin; clonidine
  • Vaginal atrophy: Topical vaginal estrogen (estriol cream)

13. PEDIATRIC OPD (Common Presentations)

Fever in Children

  • Paracetamol 15 mg/kg/dose Q4-6H; or Ibuprofen 10 mg/kg Q6-8H (>6 months)
  • Tepid sponging (lukewarm water)
  • Adequate hydration
  • Identify and treat underlying cause

Acute Diarrhea in Children

  • ORS (WHO formula) - mainstay
  • Zinc 10-20 mg/day x 10-14 days (WHO recommendation - reduces duration and severity)
  • Breastfeeding should continue
  • Antibiotics only for specific bacterial causes (cholera, dysentery)
  • Probiotics (Lactobacillus GG) - modest benefit

Respiratory Tract Infections in Children

  • Viral URTI: supportive (saline nasal drops, paracetamol)
  • Acute Otitis Media: Watchful waiting 48-72h in mild cases (>2 years); Amoxicillin 40-90 mg/kg/day if ≥6 months with severe or bilateral AOM
  • Pneumonia (outpatient, non-severe): Amoxicillin 40-90 mg/kg/day x 5-7 days
