Both, depending on the vascular bed:
  • Systemic circulation: hypoxia usually causes vasodilation to increase local blood flow and oxygen delivery, particularly in the brain and exercising muscle.
  • Pulmonary circulation: alveolar hypoxia causes vasoconstriction. This diverts blood away from poorly ventilated alveoli toward better-ventilated areas, improving V/Q matching.
