Important Notes
- Dose adjustment required in renal impairment (reduce dose if CrCl <30 mL/min)
- Use with caution in children <18 years - risk of tendon rupture, especially with concomitant corticosteroids
- Drug interactions: Inhibits CYP1A2 - increases toxicity of caffeine, theophylline, methotrexate, tizanidine
- Avoid antacids, iron, calcium supplements within 2 hours (reduce absorption)
- Systemic fluoroquinolones carry an FDA warning for disabling side effects: tendinopathy, peripheral neuropathy, CNS effects, photosensitivity
