5. Signs & Symptoms

Classic Presentation

"First-step pain" is the pathognomonic symptom:
  • Severe, sharp, stabbing pain on the plantar surface of the heel on taking the first few steps in the morning
  • Pain is worst after prolonged inactivity (after sleep, after prolonged sitting)
  • Pain characteristically improves with initial ambulation (as the fascia warms up and stretches)

  • Goals: Rule out calcaneal stress fracture, subtalar arthritis, tumor, foreign body
