Extended Infusion Option (if used at your institution):
If your hospital uses extended-infusion Piptaz (e.g., 3.375g over 4 hours every 8 hours), this is also appropriate at eGFR 46 without adjustment, and offers pharmacokinetic advantages for higher MIC organisms.
Bottom line: At eGFR 46, full standard dosing applies - no reduction is required. Dose adjustment kicks in only when eGFR drops below 40 mL/min.
