In heart failure, symptoms mainly reflect where blood backs up:
Predominant left-sided heart failurePredominant right-sided heart failure
Blood backs up into the lungsBlood backs up into the systemic veins and organs
Breathlessness on exertion is often the earliest symptomLeg and ankle swelling (peripheral edema)
Breathlessness when lying flat (orthopnea), needing extra pillowsAbdominal swelling from ascites or fluid retention
Waking suddenly at night gasping for air (paroxysmal nocturnal dyspnea)Rapid weight gain from retained fluid
Persistent cough or wheeze, sometimes with white or pink frothy sputum in severe pulmonary edemaAbdominal fullness, reduced appetite, nausea
Tiredness, poor exercise capacity, weaknessLiver congestion/enlargement, sometimes right-upper-abdominal discomfort
Reduced oxygen delivery may cause cool extremities, dizziness, confusion, reduced urine output in severe casesDistended neck veins and sometimes pleural effusions
Why this happens
  • Left-sided failure: the left ventricle cannot pump efficiently into the body, so pressure builds backward into pulmonary veins. This produces pulmonary congestion and fluid in the lungs, hence dyspnea, cough, orthopnea, and nighttime breathlessness. - Robbins & Kumar Basic Pathology, p. 309
  • Right-sided failure: the right ventricle cannot pump effectively into the lungs, so blood accumulates in systemic and portal veins. This causes edema, ascites, liver and spleen enlargement, and neck-vein distension. Pure right-sided failure usually has fewer respiratory symptoms. - Robbins & Kumar Basic Pathology, p. 311
In practice, the two often coexist: left-sided failure can raise pressure in the pulmonary circulation and eventually strain the right ventricle. This is why many people develop both breathlessness and edema. The American Heart Association symptom guide also lists fatigue, swelling, nausea/loss of appetite, palpitations, and impaired thinking among possible heart-failure symptoms.
Seek emergency care now for severe or sudden breathlessness, chest pain, fainting, blue/gray lips, confusion, or coughing up pink frothy sputum. These can indicate acute pulmonary edema or another cardiac emergency.
