What Was Done

A plain CT scan of the brain (no contrast dye) was performed - serial cross-sectional images taken from the base of the skull to the top (vertex). This is a standard emergency scan to look for bleeding, swelling, or fractures.

Findings - Explained Simply

🔴 1. Brain Bleed (Most Serious Finding)

"Left-sided capsulo ganglionic hemorrhage measuring 1.3 x 1.7 cm with surrounding minimal edema"
  • There is a bleed (hemorrhage) inside the brain on the left side, measuring about 1.3 x 1.7 cm - roughly the size of a large grape seed.
  • It is located in the basal ganglia and internal capsule - deep structures that control movement, coordination, and sensation for the opposite (right) side of the body.
  • There is a small amount of swelling (edema) around the blood clot, which is a normal reaction.
  • What this can cause: Weakness or paralysis of the right arm/leg, numbness on the right side, difficulty speaking (if left hemisphere is dominant, which it is in most people), facial drooping on the right.

🟠 2. Cheekbone Fracture (Right Side)

  • The right cheekbone (zygomatic arch) is broken and the bone fragments have shifted out of position.
  • This is a common injury in facial trauma (e.g., from a punch, fall, or accident).

🟡 3. Right Cheek Sinus Fractures + Blood in Sinus

"Hyperdense collection in right maxillary sinus - Hemosinus" + "Displaced fractures of anterior and lateral walls of right maxillary sinus"
  • The right maxillary sinus (air-filled cavity inside the right cheek) has two fractured walls - front and side.

  • It is not immediately dangerous but confirms the extent of trauma.

What Needs to Happen Urgently

  1. Neurosurgery consult - The brain bleed must be monitored. Depending on the patient's symptoms and whether it is expanding, surgery or conservative management (close monitoring, blood pressure control, etc.) may be needed.
  2. Maxillofacial surgery consult - The displaced cheekbone and sinus fractures will need surgical repair once the patient is neurologically stable.
  3. Watch for danger signs: Worsening headache, increased confusion, new weakness or numbness (especially on the right side), difficulty speaking, repeated vomiting, or loss of consciousness - these signal the bleed may be enlarging and need immediate emergency intervention.

🚨 THIS IS A MEDICAL EMERGENCY

If the patient is unconscious RIGHT NOW:


✅ DO THIS IMMEDIATELY:

  1. Call emergency services NOW - Dial 108 (India ambulance) immediately
  2. Do NOT move the patient unless they are in danger - there may be spinal injury
  3. Check if they are breathing - look for chest rise, listen for breath sounds
  4. Place them in the recovery position (on their side) if they are breathing but unconscious - this prevents choking if they vomit
  5. Do NOT give food or water
  6. Keep them warm with a blanket
  7. Stay with them and talk to them even if they don't respond

⚠️ Why unconsciousness is extremely dangerous HERE:

The CT scan showed a brain bleed in the left basal ganglia (1.3 x 1.7 cm). Unconsciousness means:
  • The bleed is likely expanding and pressing on the brain
  • Brain pressure (intracranial pressure) is rising dangerously
  • The brain may be herniating (shifting due to swelling) - this is life-threatening
  • Every minute counts - brain cells are dying

🏥 At the Hospital - What doctors will do:

  • Immediate repeat CT scan to check if the bleed has grown
  • Intubation (breathing tube) to protect the airway
  • IV medications to reduce brain swelling (Mannitol, hypertonic saline)
  • Blood pressure control
  • Neurosurgery evaluation - may need emergency surgery to remove the clot
  • ICU admission
