Diagnostic value of IgE testing

1. Total serum IgE

Total IgE is a nonspecific marker. It can support, but does not establish, an allergic diagnosis.
  • A raised result is compatible with atopy, such as allergic rhinitis, asthma, eczema, or food allergy.
  • A normal total IgE does not exclude IgE-mediated allergy.
  • It has limited diagnostic usefulness for routine adult respiratory allergy because levels overlap substantially between allergic and non-allergic people.
  • Elevated total IgE also occurs with parasitic infection, atopic dermatitis, allergic bronchopulmonary aspergillosis (ABPA), some immunodeficiencies including hyper-IgE syndromes, and occasionally malignancy or other inflammatory disorders.
Thus, do not use total IgE alone to diagnose “allergy.”

2. Allergen-specific IgE

Serum allergen-specific IgE is more diagnostically useful. It identifies sensitization to a particular allergen, such as house-dust mite, pollen, milk, peanut, egg, or a drug.
However:
  • A positive specific-IgE result means sensitization, not necessarily clinical allergy.
  • It must match the clinical history, especially reproducible symptoms after exposure.
  • The size of the result may help estimate the probability of clinical reactivity for some allergens, but does not reliably predict reaction severity.
  • Skin-prick testing and serum specific-IgE testing are complementary. Either can be appropriate depending on medications, skin disease, age, access, and safety considerations.

3. Situations where total IgE is particularly useful

  • ABPA: usually markedly elevated and helpful in diagnosis and monitoring.
  • Hyper-IgE syndrome: very high levels, often in the thousands of kU/L, together with eczema, eosinophilia, recurrent skin and lung infections.
  • Parasitic disease: may support the diagnosis in an appropriate epidemiological and clinical setting.
  • Biologic therapy assessment: total IgE is used for eligibility and dose calculation for omalizumab, not as an allergy diagnostic test.

Practical interpretation

History of exposure and symptoms first, targeted specific-IgE or skin-prick testing second, total IgE only as supportive context. Avoid broad allergy panels without a compatible history because they commonly detect irrelevant sensitization.
The medical textbook evidence gathered here states that elevated total IgE can support allergic disease, but normal values do not rule it out, and that total IgE has limited diagnostic value in adults with suspected allergy. The recent systematic review on food allergy test accuracy also supports interpreting IgE tests in the clinical context rather than treating a positive test as definitive disease (PMID: 38009299).
