Case: Epididymitis A 24-year-old male presents with a 2-day history of gradually increasing pain and swelling of the right scrotum. The pain started in the lower part of the scrotum and gradually increased. He also complains of dysuria, increased urinary frequency, and low-grade fever. On examination, the right hemiscrotum is swollen, warm, and tender, with maximum tenderness over the epididymis. The pain is relieved slightly when the scrotum is elevated. There is no history of trauma. Investigations: Urinalysis: increased WBCs Urine culture: may show bacterial growth Urethral/urine NAAT: may be performed for Chlamydia trachomatis and Neisseria gonorrhoeae Scrotal ultrasound with Doppler: enlarged, hypervascular epididymis with preserved testicular blood flow
