case 2 - 22 year old female student by occupation c/o pain in ano during defaecation , associated with pus discharge from ano occasionally since 2 years , no relavant past history , normosthenic individual , vitally stable , on inspection no evident ext opening or induration present, internally swelling present near the rectal wall , above anorectal junction - 5 to 7, o clock , collection felt internally , temp was raised , tenderness present , pus on fingerstall , investigation done was mri of perianal region revealed faintly visualized suspicious internal opening at 6' o clock of mid anal canal with a smaller calibered intersphincteric tract extending from 5-7' o clock position bifurcating into superior and inferior inter sphincteric tracts. superior secondary inter sphincteric tract reach upto the level of loer rectum with small intramural collection in left rectal wall . inferior secondary tract extending inferiorly upto the level of anal verge with localized suspicious external opening . Procedure done was partial fistulectomy with plain thread ligation from 6- ' o clock , a window made at 6 , o clock below intersphincteric groove , probing done from external wound , it came out through 6' o clock internal opening at the level of dentate line , the the wound was deepened 5 , o clock cpllection adressed a nylon mesh placed internally towards 5' oclock in the left rectal wall , post operative regimen was same as that of the first patient
