What are Ceruminolytics?

Types of Ceruminolytic Ear Drops

1. Sodium Bicarbonate (5% or 10% solution)

• Most effective ceruminolytic based on evidence - disintegrates cerumen in approximately 90 minutes vs. 18+ hours for most other agents
• Available OTC and can be prepared as a simple solution
• Preferred agent when speed of action is needed
• Often used in clinical settings before syringing

2. Docusate Sodium (Docusate / Colace liquid)

• A stool softener that doubles as a highly effective ceruminolytic
• Evidence-based reviews found docosate sodium administered 15 minutes before irrigation was the most effective agent for facilitating cerumen removal
• Available as a liquid preparation; less commonly used specifically as an ear drop but well-supported by evidence

3. Olive Oil / Almond Oil (Oil-based drops)

• Classic, widely used home remedy
• Safe, well-tolerated, and gentle
• Less effective than sodium bicarbonate but recommended for regular use over several days (usually 3-5 days) to soften wax before a GP or clinic appointment
• Preferred for patients with sensitive ears or children

4. Hydrogen Peroxide (3% solution)

• Commonly used ceruminolytic - the fizzing action helps break up wax
• Widely available OTC (e.g., Debrox contains carbamide peroxide, which releases hydrogen peroxide)
• 5-10 drops in the affected ear, twice daily, for up to 4 days
• Not as well-studied systematically, but clinically popular
• Avoid if there is any suspicion of a perforated eardrum

5. Carbamide Peroxide (Urea Hydrogen Peroxide)

• Common OTC product (Debrox, Murine Ear Drops)
• Releases oxygen bubbles that mechanically loosen wax
• Widely available and safe for short-term use (up to 4 days)

6. Water / Saline / Sodium Chloride Drops

• A Cochrane review found that any type of ear drop, including plain water and saline, is more effective than no treatment
• Warm water or saline softens wax and is safe, readily available, and low-cost
• Especially useful before irrigation

7. Triethanolamine (Cerumenex) and Cerumol

• Previously widely used oil-based commercial preparations
• Both have been removed from the market due to otic cell toxicity
• No longer recommended
