A mesh graft is a split-thickness skin graft passed through a mesher to create small openings, allowing it to be expanded before application to a burn wound.

Uses in burn patients

  • Coverage of large deep partial-thickness and full-thickness burns after excision.
  • Particularly useful when the available donor skin is limited.
  • Can be expanded in ratios such as 1:1.5 to 1:9, depending on the wound and donor-site availability. Fischer's Mastery of Surgery, p. 7672.

Advantages

  1. Covers a larger wound area
    A small donor graft can cover a much wider burned surface. This conserves scarce donor skin.
  2. Drains blood and serous fluid
    The mesh openings allow egress of fluid, reducing hematoma or seroma formation beneath the graft and improving graft take.
  3. Conforms to irregular surfaces
    It adapts well over curved or uneven areas such as the trunk, limbs, joints, and circumferential wounds.
  4. Allows wound inspection
    The grafted area can be observed through the interstices for infection, fluid collection, and graft adherence.
  5. Facilitates rapid wound coverage
    In extensive burns, early expanded autografting helps obtain coverage despite limited donor skin.
  6. Useful in heavily exudative wounds
    The openings prevent fluid trapping under the graft.
Main disadvantage: the mesh pattern may remain visible, with poorer cosmetic appearance and potentially greater contraction than a sheet graft. Therefore, sheet grafts are preferred for cosmetically important areas such as the face, hands, and neck when adequate donor skin is available.
