This is an emergency for a dialysis access.
Bleeding from a fistula puncture site for 2 days is not normal and can become life-threatening fast, especially in a dialysis patient. Go to the nearest Emergency Department now or call emergency services now.
What to do right now while going:
Use direct fingertip pressure exactly on the needle hole (not broad palm pressure), continuously for at least 10 to 15 minutes without checking.
Keep the access arm elevated.
If blood soaks through, add gauze on top and keep pressing, do not remove the first layer repeatedly.
Do not apply a tight tourniquet around the arm.
Take someone with the patient and bring dialysis details/medication list.
Important risk factors here:
Blood thinners (heparin from dialysis, aspirin, clopidogrel, warfarin, apixaban, etc.)
Very high fistula pressure / outflow stenosis
Infection or pseudoaneurysm at puncture site
Low platelets or clotting problem (common in kidney failure)
At hospital they may need urgent vascular access evaluation and blood tests (CBC/coagulation), and treatment by nephrology/vascular team.
If you want, I can give a very short handoff note you can read to triage staff immediately.
