For dark pimple marks, use a gel or cream, not a heavy ointment.
Best first option:
  • Azelaic acid 10% gel/cream: apply a thin layer at night, starting 3 nights per week, then daily if your skin tolerates it. It can help acne and fade the dark marks.
Most important in the morning:
  • Broad-spectrum sunscreen SPF 30 or higher, every day. Without sunscreen, marks become darker and take longer to fade.
Tips:
  • Do not squeeze or scrub pimples.
  • Use a gentle moisturizer if there is dryness or burning.
  • Results usually take 6-12 weeks.
  • If you are pregnant or trying to conceive, azelaic acid is generally preferred over retinoid creams. Avoid adapalene/tretinoin unless a clinician says it is appropriate.
If you still have active pimples, azelaic acid can be a useful single product. If marks are not improving after about 3 months, or you have pits/indentations rather than flat dark spots, see a dermatologist. The AAD acne advice also notes that azelaic acid can fade dark spots after acne.
