🏥 Gastritis साठी Nursing Procedures (Nursing Care)

1. 📋 Assessment (मूल्यांकन)

Subjective Data (रुग्णाकडून माहिती):
  • Epigastric pain / burning sensation (पोटाच्या वरच्या भागात दुखणे)
  • Nausea, vomiting (मळमळ, उलटी)
  • Loss of appetite (भूक न लागणे)
  • Bloating, belching (पोट फुगणे)
Objective Data (तपासणी):
  • Vital signs monitor करा - BP, pulse, temperature, RR
  • Abdomen tenderness तपासा (epigastric area)
  • Stool / vomit मध्ये blood आहे का ते बघा (hematemesis/melena)
  • Skin turgor (dehydration साठी)

3. 🔧 Nursing Procedures & Interventions

A) Pain Management

  • Prescribed antacids, PPIs (Proton Pump Inhibitors), H2 blockers (Omeprazole, Pantoprazole, Ranitidine) वेळेवर द्या
  • रुग्णाला अर्ध-उठलेल्या स्थितीत (semi-Fowler's position - 30-45°) ठेवा - acid reflux कमी होतो
  • Pain scale (0-10) वापरून दर shift ला pain assess करा
  • Warm compress epigastric area वर द्या (आराम मिळतो)

B) NPO / Diet Management

  • Acute gastritis मध्ये NPO (Nothing Per Oral) ठेवा - stomach ला rest द्या
  • IV Fluids द्या (Normal Saline / Ringer's Lactate) - hydration राखा
  • Nausea कमी झाल्यावर clear liquids सुरू करा
  • हळूहळू bland diet द्या (मसालेदार, तळलेले, caffeine-युक्त पदार्थ टाळा)

C) Nasogastric Tube (NGT) - आवश्यकतेनुसार

  • Severe vomiting किंवा bleeding असल्यास NGT insert करा
  • Gastric lavage (stomach washing) - acute hemorrhagic gastritis मध्ये
  • NGT द्वारे medications द्या जर oral route शक्य नसेल

D) IV Access & Fluid Management

  • IV cannula (IV line) घाला
  • Ordered IV fluids rate प्रमाणे द्या
  • Intake & Output (I&O) chart ठेवा - dहydration monitor करा
  • Electrolytes monitor करा (Lab values - K⁺, Na⁺)

E) Medication Administration

  • Stool guaiac test करा (occult blood तपासा)
  • Blood investigations - CBC, serum electrolytes, ABG (गरज असल्यास)

G) Rest & Comfort

  • रुग्णाला physical आणि emotional rest द्या
  • Stress कमी करा - gastritis stress मुळे वाढतो
  • Quiet environment maintain करा

4. 📚 Patient Education (रुग्ण शिक्षण)

हे nursing चे महत्त्वाचे काम आहे:
  • Alcohol आणि smoking टाळा - gastric mucosa irritate होतो
  • NSAIDs (Aspirin, Ibuprofen) टाळा - stomach lining खराब करतात
  • मसालेदार, तळलेले, caffeine असलेले पदार्थ कमी करा
  • छोटे-छोटे जेवण दिवसात 5-6 वेळा खा
  • Prescribed medications वेळेवर घ्या - course पूर्ण करा
  • Follow-up visits ला नक्की या
  • H. pylori infection असल्यास पूर्ण antibiotic course घ्या
  • Chronic gastritis मध्ये gastric cancer screening साठी endoscopy करावी

5. ✅ Evaluation (मूल्यमापन)

Nursing care यशस्वी झाली का ते बघा:
  • Pain कमी झाला का? (NRS score कमी झाला का?)
  • Vomiting थांबली का?
  • Fluid balance adequate आहे का?
  • रुग्णाला disease आणि diet बद्दल माहिती झाली का?
  • Medications घेण्याची compliance आहे का?
थोडक्यात: Gastritis nursing मध्ये मुख्यतः pain management, NPO + IV fluids, medication administration (PPIs/antacids), vital signs monitoring, NGT (गरज असल्यास), आणि patient education या पाच गोष्टी सर्वात महत्त्वाच्या आहेत. H. pylori-related gastritis असल्यास antibiotic triple therapy चे पालन करणे विशेष महत्त्वाचे आहे.
