Aaj eye ko ek camera ki tarah samjho.
- Cornea + lens = camera ka focusing system
- Iris + pupil = camera ka aperture, jo light control karta hai
- Retina = camera ka film/sensor
- Optic nerve = wire/cable
- Brain = actual processor, jo image ko samajhta hai
Light eye ke andar jaati hai, retina par focused image banti hai, retina signal banata hai, optic nerve signal brain tak le jaati hai. Brain us signal ko “image” ke roop mein interpret karta hai.

1. Eye ki basic position and size
- Eyeball bony orbit ke andar placed hota hai.
- Orbit skull ki cavity hai, jo eye ko trauma se protection deta hai.
- Adult eyeball almost spherical hota hai.
- Eye ko move karne ke liye six extraocular muscles hote hain.
- Eye ke surrounding protective structures ko adnexa of eye bolte hain: eyelids, conjunctiva, lacrimal apparatus, extraocular muscles and orbit.
2. Eyeball ke 3 coats or layers
Ye exam ka very important point hai.
Eyeball ki wall ko outside se inside tak 3 layers mein divide karte hain:

Layer 1: Outer fibrous coat
A. Sclera
- Sclera eye ka white, tough, opaque outer covering hai.
- Ye eyeball ke posterior five-sixths part ko cover karta hai.
- It gives shape and protection to the eye.
- Extraocular muscles sclera par attach hote hain, isliye eyeball movements possible hote hain.
- Posteriorly, optic nerve sclera ko pierce karke brain ki taraf jata hai.
Clinical link:
Sclera ka inflammation = scleritis, jisme severe deep eye pain and redness ho sakti hai.
B. Cornea
- Cornea eye ke front ka transparent, dome-shaped portion hai.
- Ye sclera ke anterior one-sixth part ko replace karta hai.
- Light sabse pehle cornea se enter karti hai.
- Cornea light ko bend karta hai, jise refraction bolte hain. It is the major refracting surface of the eye.
- Cornea normally blood vessels ke bina hota hai, yani avascular, isliye transparent rehta hai.
- Isme sensory nerve supply rich hoti hai, mainly trigeminal nerve ke ophthalmic division se. Isliye corneal injury bahut painful hoti hai.
Clinical link:
- Corneal ulcer can threaten vision.
- Contact lens hygiene poor ho to infection ka risk badhta hai.
- Corneal opacity se vision reduce ho sakti hai.
Layer 2: Middle vascular coat, also called Uvea
Uvea ke three parts hote hain:
- Choroid
- Ciliary body
- Iris

A. Choroid
- Choroid sclera aur retina ke beech ki dark, vascular layer hai.
- Isme abundant blood vessels hote hain.
- Main function: outer retina ko oxygen and nutrients provide karna.
- Iska dark pigment light ko absorb karta hai, so that light eye ke andar reflect na ho aur image clear bane.
Clinical link:
Choroid and adjacent tissues ki inflammation ko broadly uveitis ke context mein discuss kiya jata hai.
B. Ciliary body
Ciliary body iris ke just piche present hota hai. Iske two major functions hain:
1. Aqueous humour banana
Ciliary processes clear fluid banate hain called aqueous humour.
2. Lens ka shape change karna
Ciliary muscle zonular fibers ke through lens ko control karta hai.
Accommodation kya hai?
Near object dekhte waqt:
- Ciliary muscle contracts
- Suspensory ligaments relax hote hain
- Lens more rounded/thick ho jata hai
- Near object ka light retina par focus hota hai
Far object dekhte waqt:
- Ciliary muscle relaxes
- Ligaments tight hote hain
- Lens flatter ho jata hai
Clinical link:
Age ke saath lens less elastic hota hai, near vision difficult hoti hai. Isse presbyopia kehte hain.
C. Iris
- Iris eye ka colored part hai.
- Brown, black, blue etc. eye colour iris ke pigment par depend karta hai.
- Iris ke center mein opening hoti hai called pupil.
- Iris pupil ka size control karke eye mein entering light regulate karta hai.
Iris muscles

Easy concept:
- Bright light → pupil small
- Dark room → pupil large
Clinical link:
- Unequal pupils = anisocoria
- Fixed dilated pupil can be an emergency sign in neurological illness or acute glaucoma context.
- Nurses assess pupil size, equality and reaction to light.
3. Pupil
Pupil khud koi structure or muscle nahi hai. It is simply a central opening in iris.
Think: Room ki window mein curtain iris hai; curtain ke beech ka open hole pupil hai.
Function: Eye mein kitni light enter hogi, ye decide karta hai.
4. Lens
- Lens iris aur pupil ke behind situated hota hai.
- It is transparent, biconvex and elastic.
- Lens ka main work light ko retina par focus karna hai.
- Lens is held in position by suspensory ligaments or zonules, which attach it to the ciliary body.
- Lens avascular hai and gets nutrition from aqueous humour and vitreous.
Clinical link: Cataract
Lens normally clear hota hai. Jab lens opaque/cloudy ho jata hai, it is called cataract.
Common symptoms:
- Blurred vision
- Glare, especially at night
- Halos around lights
- Gradual painless decrease in vision
5. Chambers of the eye and fluids
Eye ke anterior part mein aqueous humour, aur posterior large cavity mein vitreous humour hota hai.
A. Anterior chamber
- Located between cornea and iris.
- Isme aqueous humour present hota hai.
B. Posterior chamber
- Located behind iris and in front of lens/ciliary body.
- This is a small space, not the same as “posterior segment.”
C. Aqueous humour: flow and drainage
Aqueous humour ciliary processes banate hain.
Flow yaad rakho:
Ciliary body → posterior chamber → pupil → anterior chamber → trabecular meshwork → canal of Schlemm → veins
Aqueous humour ka function:
- Cornea and lens ko nutrition dena
- Waste remove karna
- Intraocular pressure maintain karna
- Eye ka shape maintain karna
Clinical link: Glaucoma
Jab aqueous humour properly drain nahi hota, intraocular pressure may rise. This can damage optic nerve and cause irreversible visual loss.
D. Vitreous chamber and vitreous humour
- Lens ke piche se retina tak ka large posterior cavity = vitreous chamber.
- It is filled with a transparent jelly-like material called vitreous humour.
- Vitreous humour aqueous humour jaisa continuously circulate nahi karta.
Functions:
- Eyeball ka shape maintain karna
- Retina ko support dena
- Light ko retina tak pass hone dena
Clinical link:
Floaters, flashes of light, or a “curtain-like” shadow can suggest vitreous or retinal pathology and needs urgent eye assessment.
6. Retina: The screen of eye
Retina eye ki innermost, light-sensitive nervous layer hai.
Main work:
Light energy → electrical nerve impulse
Retina mein special receptor cells hote hain:

Macula
- Retina ka central sensitive area.

Fovea centralis

Optic disc
- Jahan optic nerve retina se exit karta hai.
- Yahan rods and cones absent hote hain.
- Therefore it is called blind spot.
Clinical link:
- Diabetic retinopathy retina ki blood vessels ko damage karti hai.
- Macular disease se central vision affect hoti hai.
- Retinal detachment can cause sudden painless visual loss.
7. Optic nerve
- Optic nerve is cranial nerve II.
- It carries visual impulses from retina to brain.
- Retina light ko signals mein change karti hai; optic nerve signal ko brain ke visual cortex tak le jata hai.
- Brain signal ko process karke meaningful image banata hai.
So remember:
Eye receives light, retina converts it into impulse, optic nerve carries it, brain sees it.

8. Extraocular muscles: Eye ko move kaun karata hai?
There are six muscles:
- Superior rectus
- Inferior rectus
- Medial rectus
- Lateral rectus
- Superior oblique

LR6 SO4, all others 3
- Lateral rectus: CN VI, abducens nerve
- Superior oblique: CN IV, trochlear nerve
- All other extraocular muscles: CN III, oculomotor nerve
Simple functions:
- Medial rectus: eye inward
- Lateral rectus: eye outward
- Superior rectus: upward
- Inferior rectus: downward
9. Protective structures of eye
Eyelids
- Eyelids protect eye from trauma, dust and excessive light.
- Blinking spreads tears across cornea and prevents dryness.
Eyelashes
- Dust/foreign particles ko stop karne mein help karti hain.
Conjunctiva
- Thin transparent mucous membrane.
- It covers inner eyelid surface and front of sclera.
- It does not cover cornea.
- Infection/inflammation of conjunctiva = conjunctivitis.
Lacrimal apparatus
Tears banane aur drain karne wala system.
Flow:
Lacrimal gland → tear film over eye → puncta → canaliculi → lacrimal sac → nasolacrimal duct → nose
Isliye crying ke time nose bhi run hota hai.
Tears ke functions:
- Eye ko moist and lubricated rakhna
- Foreign particles ko wash out karna
- Infection se protection, due to antimicrobial substances such as lysozyme

Eye assessment ke time nurse ko observe karna chahiye:
- Visual acuity
- Eye redness, swelling, discharge
- Eyelid and conjunctiva
- Pupil size, equality, shape and reaction to light
