Pneumocystis pneumonia AND treatment

NIH HIV opportunistic infection guidelines Pneumocystis pneumonia TMP-SMX corticosteroids prophylaxis

Task 1

  1. Diagnosis: Pneumocystis jirovecii pneumonia (PJP/PCP), a severe atypical interstitial pneumonia.
  2. Confirmation: Induced sputum or bronchoalveolar lavage (BAL) with P. jirovecii PCR and immunofluorescent staining.
  3. Treatment: IV co-trimoxazole (TMP 15-20 mg/kg/day) for 21 days plus corticosteroids because PaO₂ is 68 mmHg.

Task 2

  1. Characteristic laboratory findings:
    • Hypoxemia: PaO₂ 62 mmHg with respiratory alkalosis (low PaCO₂).
    • Raised serum LDH: 480 U/L.
  2. Why high risk: Cyclophosphamide and prolonged prednisolone cause impaired cellular immunity.
  3. Severe PCP dose: IV TMP-SMX: trimethoprim 15-20 mg/kg/day plus sulfamethoxazole 75-100 mg/kg/day, in divided doses, for 21 days.

Task 3

  1. If co-trimoxazole allergy: Use IV pentamidine 4 mg/kg once daily. An alternative is clindamycin plus primaquine if appropriate.
  2. Additional treatment for severe hypoxia: Adjunctive corticosteroids, for example prednisone 40 mg twice daily initially, started within 72 hours.
  3. Secondary prophylaxis: Start immediately after completing the 21-day treatment course; continue during immunosuppression until immune recovery.
These regimens agree with the NIH PCP guideline.

Task 1

  1. Diagnosis: Pneumocystis jirovecii pneumonia (PCP/PJP).
  2. Confirmatory test: BAL fluid PCR for Pneumocystis jirovecii.
  3. Treatment: IV co-trimoxazole for 21 days + corticosteroids because PaO₂ is <70 mmHg.

Task 2

  1. Two characteristic lab findings: Low PaO₂ (hypoxemia) and high LDH.
  2. Reason for high risk: Prednisolone and cyclophosphamide suppress cellular immunity.
  3. Severe PCP dose: TMP-SMX IV: TMP 15-20 mg/kg/day + SMX 75-100 mg/kg/day for 21 days.

Task 3

  1. If co-trimoxazole allergy: IV pentamidine.
  2. For severe hypoxia: Add corticosteroids.
  3. Secondary prophylaxis: Start after finishing 21 days of treatment and continue until immune recovery.
