कुठे वापरतात?

  • Mania आणि suicide चा high risk असलेल्या cases मध्ये पण वापरतात.
फायदे (Advantages):

तोटे (Adverse Effects):
  • Memory loss होऊ शकतो - विशेषतः treatment आधीच्या काही महिन्यांच्या आठवणी जाऊ शकतात (retrograde amnesia).
  • नवीन माहिती temporarily store करणे पण कठीण होऊ शकते.
Mechanism कसा काम करतो?
  • नक्की mechanism अजूनही पूर्णपणे माहित नाही, पण असे मानतात की ECT brain मधील synapses ला "reboot" करते, विशेषतः hippocampus आणि HPA axis वर परिणाम करते.

🔴 Major Diagnostic Indications

2Mania (mixed episodes सकट)
3Schizophrenia - acute exacerbation किंवा catatonic subtype
4Schizoaffective Disorder

🟡 Other Diagnostic Indications

  • Parkinson's Disease (on-off phenomenon मध्ये)
  • Neuroleptic Malignant Syndrome (NMS)

🟢 Clinical Indications

Primary Use (पहिल्यांदाच ECT द्यायचे):
  • जेव्हा rapid response हवी असते - म्हणजे patient deteriorate होत आहे किंवा suicide चा high risk आहे
  • Medications चे risks जास्त आहेत benefits पेक्षा
  • आधी medications काम केली नव्हती, पण ECT ने बरे झाले होते
  • Patient ची स्वतःची preference
Secondary Use (medications फेल झाल्यावर):
  • Current episode मध्ये pharmacotherapy fail झाली
  • Medications tolerate होत नाहीत (side effects खूप आहेत)
  • Patient ची condition खूप खराब होत आहे

🔵 Special Situations - ECT हे best option आहे:

  • Pregnant women ज्यांना medications घेता येत नाहीत + suicidal आहेत
  • Geriatric patients जे antidepressants safely घेऊ शकत नाहीत
  • Suicidal children/adolescents जे medications ला respond करत नाहीत
  • Catatonia, OCD, intractable seizure disorders

❌ ECT काम करत नाही:

  • Personality disorders
  • Anxiety disorders
  • Somatic symptom disorder (जोपर्यंत depression सोबत नाही)
