ADA tooth decay treatment fillings crowns root canal extraction wisdom teeth

Arrange a dental appointment soon, ideally within a few days. A cavity in a wisdom tooth can spread or cause infection, and a tooth with a cap (crown) can still develop decay at the edge or underneath it.
What the dentist will do:
  • Examine both sides and take X-rays if needed.
  • Wisdom tooth: If it is reachable, useful for chewing, and decay is small, it may be filled. If it is badly decayed, difficult to clean, partly impacted, or repeatedly infected, removal is often the better option.
  • Crowned tooth: The crown may be fine, but decay can occur around its margin. Treatment may be a filling at the edge, crown replacement, or a root canal plus a new crown if the nerve is affected. It cannot be decided without an exam/X-ray.
Until you are seen:
  • Brush twice daily with fluoride toothpaste, including around the crown and at the very back.
  • Floss or use an interdental brush around the crown if it does not dislodge it.
  • Limit sugary drinks/snacks and avoid chewing hard or very sweet foods on the painful side.
  • For pain, use an over-the-counter pain medicine only if you normally can take it and follow the package directions. Do not place aspirin directly on the gum or tooth.
Seek urgent dental care today if you have facial swelling, fever, pus/bad taste, trouble opening your mouth, swallowing, or breathing, or severe worsening pain. Cavities with dentine involvement generally need professional removal of decay and restoration, rather than home treatment (Scott-Brown's Otorhinolaryngology Head & Neck Surgery, p. section Table 42.1). The ADA notes that severity determines whether a filling, crown, root canal, or extraction is needed (ADA oral-health guidance).
