INTRODUCTION

Screening is defined as "the presumptive identification of unrecognised disease or defect by the application of tests, examinations or other procedures which can be applied rapidly."
A screening test is NOT a diagnostic test β€” it only separates those who probably have the disease from those who probably do not.
The test must satisfy the criteria of:
  1. Acceptability
  2. Repeatability
  3. Validity (Accuracy)
  4. Yield, Simplicity, Safety, Cost

1. ACCEPTABILITY (2 marks)

  • Since a high rate of cooperation is essential, the test must be acceptable to the population at whom it is aimed
  • Tests that are painful, discomforting or embarrassing (e.g., rectal or vaginal examination) are NOT likely to be acceptable in mass campaigns
  • The test should be:
    • Simple and quick to perform
    • Non-invasive wherever possible
    • Culturally acceptable to the community
    • Affordable and easily available

  • Fluctuations in the variable being measured within the same individual due to:
    • (a) Changes in the parameter observed β€” e.g., cervical smear may be normal one day and abnormal another day
    • (b) Variations in how patients perceive symptoms β€” patient may give different answers on different days
    • (c) Regression to the mean β€” extreme values tend to move toward the average on repeat measurement (e.g., BP or blood glucose)
Key difference: Observer variation is checked by repeat measurements at the same time; Biological variation is tested by repeat measurements over time.

3. VALIDITY (ACCURACY) (5 marks)

Definition: "The extent to which the test accurately measures what it purports to measure." In other words, the ability of a test to separate those who have the disease from those who do not.
Example: Glycosuria is a useful screening test for diabetes, but the more valid test is the Glucose Tolerance Test (GTT).

B. SPECIFICITY

C. PREDICTIVE ACCURACY

i. Positive Predictive Value (PPV): $$\text{PPV} = \frac{a}{a+b} \times 100$$ = Probability that a person with POSITIVE test actually HAS the disease
ii. Negative Predictive Value (NPV): $$\text{NPV} = \frac{d}{c+d} \times 100$$ = Probability that a person with NEGATIVE test truly DOES NOT have the disease
