NHS muscle strain self care when seek urgent medical help arm pain

This may be a minor muscle strain from repetitive washing or lifting.
For the next 24-48 hours:
  • Rest the arm and avoid lifting, scrubbing, or overhead movements.
  • Apply a cold pack wrapped in cloth for 15-20 minutes at a time, every few hours today.
  • Gentle movement is fine, but stop if it worsens pain.
  • You can take paracetamol/acetaminophen if you normally can take it. Avoid ibuprofen if you have stomach ulcers, kidney disease, take blood thinners, or are pregnant.
Get urgent medical help now if the arm pain occurs with chest pain, shortness of breath, sweating, nausea, faintness, or pain spreading to the jaw/back, or if the arm is swollen, deformed, very weak, numb, or cold/pale.
See a clinician if it is severe, you cannot lift the arm, there is worsening bruising/swelling, or it is not starting to improve within a few days.

What gentle exercises can I do for the shoulder and upper arm?

NHS shoulder pain gentle exercises pendulum shoulder blade setting

If this began after washing clothes and there was no fall or sudden tear, start very gently. Do these 2-3 times a day, staying within a comfortable range. Mild stretching is okay, but stop if you get sharp pain, worsening pain, tingling, numbness, or weakness.
  1. Pendulum swings
    • Lean forward with your good hand supported on a table or chair.
    • Let the sore arm hang fully relaxed.
    • Gently swing it forward and back, side to side, then in small circles.
    • Do each direction for 30-60 seconds.
  2. Elbow bends
    • Keep your upper arm close to your side.
    • Slowly bend and straighten your elbow.
    • Do 10 repetitions.
  3. Shoulder-blade squeezes
    • Sit or stand tall, shoulders relaxed.
    • Gently pull your shoulder blades slightly back and down, without shrugging.
    • Hold 3-5 seconds, then relax.
    • Do 8-10 repetitions.
  4. Table slides
    • Sit facing a table with a towel under your hands.
    • Slide both hands forward slowly, allowing the painful arm to move only as far as comfortable.
    • Slide back.
    • Do 5-10 repetitions.
  5. Finger walk up a wall
    • Face a wall and use your fingers to “walk” the sore-side hand upward.
    • Go only to a mild stretch, not pain.
    • Hold 3-5 seconds, then walk back down.
    • Do 5-10 repetitions.
Avoid weights, push-ups, forceful stretching, vigorous massage, and overhead activity for now. Exercise-based rehabilitation generally focuses first on restoring comfortable shoulder movement, then later on strengthening the rotator cuff and shoulder-blade muscles. NHS shoulder exercise guidance supports pendulum and assisted movement exercises.
If you cannot raise the arm, have marked swelling/bruising, increasing pain, or symptoms are not beginning to improve after several days, arrange an in-person assessment.
