Most likely cause: massive concealed hemorrhage with hypovolemic shock, often complicated by disseminated intravascular coagulation (DIC).
In severe abruptio placentae with IUFD, placental tissue factors can trigger DIC, causing rapid consumption of fibrinogen and clotting factors. This leads to uncontrollable bleeding during or after delivery, postpartum hemorrhage, shock, renal failure, and death.
Other important differential causes of sudden maternal collapse/death include:
- Amniotic fluid embolism: sudden hypoxia, hypotension/cardiac arrest, then severe DIC
- Uterine rupture or genital tract trauma
- Severe pre-eclampsia/HELLP-related complications
- Sepsis, especially if IUFD was retained for a prolonged period
A definitive cause requires the clinical record, timing of collapse, estimated blood loss, coagulation results including fibrinogen, and postmortem findings. In an abruption with IUFD, however, DIC plus massive obstetric hemorrhage is the classic explanation.
