2024 neonatal red blood cell transfusion guideline preterm infants hemoglobin threshold respiratory support
For packed red-cell transfusion in very preterm infants (generally <30 to 32 weeks), use a restrictive Hb threshold, adjusted for postnatal age and respiratory support:

Equivalent hematocrits are approximately 32%, 29%, 25%, and 21%, respectively.
*Respiratory support includes invasive ventilation, CPAP/NIPPV, or nasal cannula flow ≥1 L/min.
These are guideline thresholds for stable very-preterm neonates, not automatic rules. A higher threshold may be appropriate with active bleeding, significant cardiorespiratory instability, sepsis/NEC with shock, severe recurrent apnea, or perioperative needs. Follow the unit's NICU protocol and clinical picture. The 2024 clinical guideline and Canadian Paediatric Society table support this approach.
