Otosclerosis
Otosclerosis is abnormal bone remodeling of the otic capsule, most often around the stapes footplate. The stapes becomes fixed, preventing efficient transmission of sound into the inner ear and causing a usually progressive conductive hearing loss. It may later involve the cochlea, giving a mixed conductive and sensorineural loss.
Typical presentation
- Gradual hearing loss, often beginning in the 20s to 40s
- Often bilateral but asymmetrical
- Tinnitus is common
- Hearing difficulty may be prominent for low frequencies
- Normal ear examination and intact tympanic membrane
- Family history is frequent, but not universal

Examination and investigations
- Otoscopy: generally normal, occasionally a reddish hue behind the tympanic membrane, called Schwartze sign
- Tuning forks: Rinne may be negative in the affected ear; Weber lateralizes to the poorer ear if loss is conductive
- Pure-tone audiogram: conductive air-bone gap, often worse at low frequencies; a Carhart notch at 2 kHz in bone conduction can occur
- Tympanometry: often reduced compliance, type As curve
- Acoustic reflexes: commonly absent
- CT temporal bone: not routinely necessary in straightforward cases but useful if the diagnosis is uncertain, disease is atypical, or surgery is being planned.

Management
Choice depends on hearing level, functional impact, audiometry, and patient preference:

Observation with repeat audiograms
Suitable for mild, stable hearing loss.

Hearing aid
A safe and effective non-surgical option for conductive or mixed hearing loss.

Stapedotomy or stapedectomy
An ENT surgeon creates/replaces the fixed stapes mechanism with a small prosthesis, usually improving the conductive component. It does not correct pre-existing cochlear sensorineural hearing loss.

Bone-conduction devices or cochlear implantation
Considered in selected advanced or surgically unsuitable cases.
Potential surgical complications include vertigo, taste disturbance, tympanic-membrane injury, tinnitus, and rare worsening or complete loss of hearing. A hearing-aid trial is a reasonable alternative before surgery. - Scott-Brown's Otorhinolaryngology, p. 488

Differentials
- Ossicular-chain fixation or discontinuity
- Chronic otitis media with ossicular disease
- Superior semicircular canal dehiscence
- Congenital stapes fixation
- Cholesteatoma or other middle-ear pathology
Seek ENT/audiology assessment for new or progressive hearing loss, unilateral symptoms, marked vertigo, facial weakness, ear discharge, or sudden hearing loss.
