American Academy Otolaryngology chronic rhinosinusitis patient information nasal saline intranasal corticosteroids
OMU means ostiomeatal unit/complex. It is the main drainage pathway that allows mucus to leave the maxillary, frontal, and anterior ethmoid sinuses into the nose.
Your CT report indicates:
Sinusitis: inflammation/swelling of the sinus lining, greatest in the right maxillary sinus, with milder changes in the left maxillary, ethmoid, and sphenoid sinuses.
OMU blockage on both sides: swelling has blocked the openings of both maxillary sinuses and the fronto-ethmoidal drainage channels. This can cause nasal blockage, facial pressure, post-nasal drip, headache, reduced smell, and recurrent sinus infections.
Deviated nasal septum to the right with a bony spur: the central wall of the nose bends rightward, which can add to obstruction.
Left inferior turbinate hypertrophy: enlargement of normal nasal tissue on the left, also contributing to nasal blockage.
No sinus bone damage: reassuring. The report does not describe bone destruction or a mass.
Keros type II and optic nerve type II: these are anatomical descriptions that are mainly important if an ENT surgeon ever considers endoscopic sinus surgery. They are not diseases by themselves.
What to do
Arrange an appointment with an ENT specialist, taking this report. Treatment often starts with saline nasal rinses and a steroid nasal spray prescribed by the clinician. Antibiotics are not automatically needed and should only be used if the ENT clinician finds a bacterial infection.
Seek urgent care if there is eye swelling/redness, reduced or double vision, severe or worsening headache, high fever, confusion, or neck stiffness.

CT PNS findings, in simple words:
Sinusitis
Swelling of the sinus lining is present.
It is severe in the right maxillary sinus.
Mild swelling is present in the left maxillary sinus, both ethmoid sinuses, and sphenoid sinuses.
Bilateral OMU blockage
The osteomeatal units (OMU) are the drainage passages of the cheek and forehead sinuses.
Both sides are blocked because of the swollen lining. This can trap mucus and maintain/recur sinusitis.
Deviated nasal septum
The partition inside the nose is bent toward the right side.
A bony spur is also present, which may worsen blockage or contact-related facial pain in some people.
Left inferior turbinate hypertrophy
The normal soft tissue inside the left lower nose is enlarged.
This can contribute to nasal obstruction, particularly if there is allergy or chronic nasal inflammation.
Spheno-ethmoid recesses are normal
The drainage region for the sphenoid sinus appears open/normal.
No destructive or alarming bone changes
Sinus walls are intact.
No bony erosion, lytic lesion, or sclerotic lesion is described. This is reassuring.
Optic nerve type II and Keros type II
These are normal anatomical variants noted mainly for ENT surgical planning.
They are not a disease or cause of symptoms.
Overall impression: Sinus inflammation with blockage of sinus drainage pathways on both sides, plus a right-sided deviated septum and enlarged left turbinate. An ENT consultation is appropriate to correlate this with symptoms and decide on medical treatment versus any procedure if symptoms persist.
