CDC perinatal hepatitis B prevention HBsAg positive mother newborn HBIG vaccine 12 hours

A baby born to an HBsAg-positive mother needs post-exposure prophylaxis immediately:
  1. Hepatitis B vaccine birth dose IM, preferably within 12 hours of birth
  2. Hepatitis B immunoglobulin (HBIG) IM, also within 12 hours of birth
    • Give vaccine and HBIG in different limbs/sites.
  3. Complete the hepatitis B vaccine series as per the local/national schedule.
    • If birth weight is <2 kg, the birth vaccine dose does not count toward the routine series, so the baby needs a total of 4 vaccine doses.
  4. At age 9-12 months, or at least 1-2 months after the final vaccine dose, test:
    • HBsAg
    • anti-HBs antibody
This combined vaccine plus HBIG approach prevents about 94% of perinatal HBV transmission according to CDC perinatal hepatitis B guidance. It is safe for the mother to breastfeed once the infant has received prophylaxis.
If the baby has not yet received vaccine/HBIG, arrange this urgently today through the newborn unit or pediatrician.
