Contraindications & Cautions in Children

  • Do not use in infants under 6 months
  • Avoid in dehydration (risk of acute kidney injury)
  • Caution with renal or hepatic insufficiency
  • Caution with known aspirin hypersensitivity (cross-reactivity)
  • Do not use with active GI bleeding or peptic ulcer disease
  • Inhibits platelet aggregation - use with caution pre-surgery
  • At >30 weeks gestation in pregnant patients: contraindicated (premature ductus arteriosus closure)
  • Avoid in varicella (chickenpox) - associated with increased risk of invasive group A streptococcal infection

Practical Tips

  1. Always use the measuring syringe or cup that comes with the medication - household spoons are inaccurate
  2. Give with food or milk to reduce stomach upset
  3. Do not give more than 4 doses in 24 hours
  4. If fever/pain is not controlled after 24-48 hours, seek medical evaluation
  5. Can be alternated with acetaminophen every 3-4 hours if needed for refractory fever, though evidence for this practice is mixed
