The heart does not pass through the lung hilum. The hilum is the gateway on the medial surface of each lung where the lung root connects it to the mediastinum.
Structures passing through the hilum are:
- Main bronchus
- Pulmonary artery
- Two pulmonary veins
- Bronchial vessels
- Lymphatics and hilar lymph nodes
- Autonomic nerves
Relation of pulmonary artery to bronchus
Use RALS:
- Right pulmonary artery is Anterior to the right main bronchus.
- Left pulmonary artery is Superior to the left main bronchus.
From anterior to posterior at either hilum, the main order is generally:
Pulmonary vein - pulmonary artery - bronchus
The pulmonary veins carry oxygenated blood from the lungs through the hila to the left atrium of the heart. The root of each lung is connected to the heart and great vessels through these pulmonary vessels. NCBI anatomy reference

At the hilum of each lung, structures either enter or leave the lung.
| Structure | Direction |
|---|---|
| Main bronchus | Enters the lung, carrying air |
| Pulmonary artery | Enters the lung, carrying deoxygenated blood from the right ventricle |
| Bronchial arteries | Enter the lung, supplying lung tissue |
| Autonomic nerves | Enter the lung via the pulmonary plexus |
| Lymphatic vessels | Mainly leave the lung, draining to bronchopulmonary (hilar) lymph nodes |
| Pulmonary veins, superior and inferior | Leave the lung, carrying oxygenated blood to the left atrium |
| Bronchial veins | Leave the lung, draining mainly to azygos or hemiazygos veins |
Easy recall:
- Enter: Bronchus + pulmonary artery + bronchial arteries + nerves
- Exit: Pulmonary veins + lymphatics + bronchial veins
The principal arrangement from anterior to posterior is:
Vein - Artery - Bronchus (VAB)
Also remember RALS:
- Right pulmonary artery is Anterior to the bronchus.
- Left pulmonary artery is Superior to the bronchus.
