The report does not establish one definite diagnosis, but it has three notable findings:
  1. Low white blood cell count (leukopenia)
    • TLC: 2,510/µL (reference 4,000-10,000)
    • Neutrophils: 47%, giving an estimated absolute neutrophil count (ANC) about 1,180/µL
    • This is mild neutropenia. It can occur temporarily with viral infections, some medicines, nutritional deficiencies, autoimmune conditions, or less commonly bone-marrow disorders.
  2. Mild microcytosis / possible early iron deficiency
    • PCV: 35.1%, low
    • MCV: 81.6 fL, slightly low
    • Hemoglobin: 12.1 g/dL, at the lower end of the lab range
      This pattern can fit early iron deficiency, including from heavy menstrual bleeding. Iron studies, especially serum ferritin, are appropriate before calling it iron-deficiency anemia.
  3. Mild isolated AST/SGOT elevation
    • AST/SGOT: 51.3 U/L (reference up to 31)
    • ALT/SGPT, bilirubin, alkaline phosphatase, albumin, creatinine, urea, platelets, and protein levels are normal.
    • AST may rise from liver irritation, recent strenuous exercise, muscle injury, alcohol, infections, or medications/supplements. It is not specific for liver disease.
What to do
  • Arrange a clinician review soon, ideally within a few days.
  • Repeat CBC with peripheral smear and differential/ANC in about 1-2 weeks, or sooner if unwell.
  • Discuss ferritin, serum iron/TIBC or transferrin saturation, and depending on history, B12/folate testing.
  • Repeat liver enzymes, ideally avoiding alcohol and strenuous exercise for several days beforehand, and review all medicines, herbal products, and supplements with the clinician.
Seek urgent care today for fever of 38°C or more, chills, mouth ulcers, severe sore throat, shortness of breath, unusual bruising/bleeding, or feeling very unwell, because a low neutrophil count can make infections more important.
