meropenem prescribing information pediatric dosage renal impairment 50 kg

For a 14-year-old weighing 40 kg, use pediatric weight-based IV dosing if renal function is normal:
IndicationDoseCalculation at 40 kgFrequency
Complicated skin/soft-tissue infection10 mg/kg400 mg IVevery 8 hours
Complicated intra-abdominal infection20 mg/kg800 mg IVevery 8 hours
Bacterial meningitis40 mg/kg1,600 mg IVevery 8 hours
Infuse over about 15-30 minutes. Do not exceed the respective maximum doses: 500 mg, 1 g, and 2 g per dose. This assumes normal kidney function. The product label notes limited experience guiding meropenem dosing in children with renal impairment, so dosing should be individualized by the treating team. See the DailyMed prescribing information.
Meropenem is a restricted broad-spectrum IV antibiotic. The exact regimen also depends on the infection, culture results, local resistance patterns, allergy history, and renal function. Check for concurrent valproate/divalproex, because meropenem can substantially lower valproate concentrations and increase seizure risk.

Recent guidance supports a maximum of 2 g IV per dose every 8 hours for pediatric meropenem in children over 3 months with normal renal function. That is a maximum total daily dose of 6 g/day.
For the 14-year-old, 40 kg patient:
Clinical useCurrent pediatric regimenDose for 40 kgMaximum per dose
Usual serious infection, including many intra-abdominal infections20 mg/kg IV q8h800 mg IV q8h2 g
Meningitis40 mg/kg IV q8h1.6 g IV q8h2 g
Severe infection or organisms with reduced susceptibility, such as PseudomonasSome institutional guidelines use 40 mg/kg IV q8h1.6 g IV q8h2 g
The current UCSF pediatric antimicrobial guideline lists 20 mg/kg/dose q8h, escalating to 40 mg/kg/dose q8h for meningitis, with a 2,000 mg/dose maximum. It recommends renal adjustment when CrCl is below 50 mL/min/1.73 m². See the UCSF pediatric dosing guideline.
The 2024 FDA labeling for pediatric bacterial meningitis likewise specifies 40 mg/kg IV q8h, maximum 2 g q8h, infused over 15-30 minutes. It states that pediatric renal-impairment data are lacking, so adjustment needs pediatric pharmacy or infectious-disease input. See the FDA label.
The UK SmPC similarly permits up to 40 mg/kg q8h in children up to 50 kg for meningitis, cystic fibrosis pulmonary exacerbations, or selected severe/resistant infections. It notes that 40 mg/kg three times daily may be appropriate for very severe infection or less-susceptible organisms. See the UK product guidance.
Important practical point: at 40 kg, a dose of 2 g q8h would be 50 mg/kg/dose, exceeding the commonly cited 40 mg/kg pediatric regimen. Do not round 1.6 g to 2 g solely for convenience unless an infectious-disease specialist or local pediatric protocol specifically directs that approach.
A recent pediatric pharmacokinetic systematic review found substantial variability in meropenem clearance driven by body weight, age, and renal function, particularly in critically ill children. This supports individualized dosing and, where available, extended infusion or therapeutic drug monitoring for severe infection, altered renal function, ECMO, or renal replacement therapy: Gao et al. (2026), systematic review, PMID: 42824418.

WHO malaria guidelines 2025 severe malaria artesunate children 20 kg 3 mg/kg 2.4 mg/kg 0 12 24 hours

Falcigo injection is artesunate. For severe malaria, use parenteral artesunate under hospital supervision.

Pediatric IV/IM artesunate schedule

Child’s weightDose per administrationSchedule
<20 kg3 mg/kg/doseAt 0, 12, and 24 hours, then once daily
≥20 kg2.4 mg/kg/doseAt 0, 12, and 24 hours, then once daily
Use IV where possible; IM may be used if IV access is unavailable. Continue until the child can take oral medication and has received at least 24 hours of parenteral treatment. Then complete treatment with a full 3-day oral artemisinin-based combination therapy (ACT). The first ACT dose should generally be given 8-12 hours after the last artesunate injection. This is consistent with the WHO malaria guideline.

For the previously mentioned 14-year-old, 40 kg girl

She is ≥20 kg:
  • Artesunate 2.4 mg/kg × 40 kg = 96 mg per dose
  • Give 96 mg IV/IM at 0, 12, and 24 hours
  • Then 96 mg once daily until oral therapy is tolerated
  • Follow with a complete 3-day ACT, selected according to local malaria guidelines, species, and resistance pattern.
Do not substitute a fixed 120 mg vial as the dose simply for convenience. Artesunate should be reconstituted and the calculated dose withdrawn, with any remainder handled according to the product instructions and local aseptic policy.
Important: This regimen is for severe or complicated malaria. For uncomplicated malaria, injectable Falcigo is not the routine regimen. Also, Falcigo-SP Kit and Falcigo Plus are different products and must not be dosed using this injection schedule.
