🦂 Bichhu ka Daank (Scorpion Sting) — Management Guide

🔴 Patient Presentation

• Right lower limb ke finger pe scorpion sting
• Turant severe pain hogi sting site pe
• Local numbness, paresthesia (tingling/jhunzhunahat) ho sakti hai
• Swelling/redness ho bhi sakti hai, nahi bhi (species pe depend karta hai)

✅ STEP 1 — First Aid / Pre-Hospital

1. Scorpion se patient ko door karo — safely, khud ko daank se bachao
2. Ice bag apply karo — sting site pe wrapped towel ke saath (direct ice skin pe nahi)
3. Affected limb ko still rakho — hilana-dulana band karo
4. Turant hospital laao — transport mein delay mat karo
5. Scorpion identify karne ki koshish karo — agar safely possible ho, photo lo (but delay mat karo)
6. Tight bandage/tourniquet NAHI lagana — harmful hota hai

✅ STEP 2 — Hospital (Emergency Department) Assessment

History Lo:

• Sting kab hua? (time)
• Kahan hua? (location — finger, confirmed)
• Scorpion kaisa dikhta tha? (color, size)
• Koi previous allergic history?
• Koi medicines chal rahi hain?

Examination:

• Sting site ko inspect karo — fang marks, redness, swelling
• Vitals monitor karo — BP, HR, SpO₂, RR
• Neurological signs dekho — roving eye movements, drooling, slurred speech, limb jerking

✅ STEP 3 — Severity Grading (Envenomation Grade)

✅ STEP 4 — Treatment

🟢 Mild Case (Grade 1 & 2):

• Local wound care — soap and water se saaf karo
• Pain relief:
  • Paracetamol / NSAIDs — mild symptoms ke liye
  • Opioid (e.g., Fentanyl/Morphine) — severe pain ke liye
• Local anesthesia infiltration — sting site pe pain management ke liye (recommended)
• Observe 4–6 hours hospital mein
• Tetanus prophylaxis — agar vaccination up-to-date nahi hai

🔴 Severe Case (Grade 3 & 4):

• Antivenom (Antivenin) — most effective intervention hai
  • India mein: Indian Scorpion Antivenom available hai
  • IV route se dena
• Benzodiazepines (Midazolam/Diazepam) — severe anxiety, muscle hyperactivity, agitation ke liye
• Prazosin — especially India mein Mesobuthus tamulus ke daank mein; catecholamine excess aur cardiovascular compromise ke liye effective
• Atropine — sirf specific cholinergic species ke liye
• Nitroglycerin — agar pulmonary edema develop ho
• Airway management — ventilation support zaroorat par, especially children mein
• IV fluids — hemodynamic support ke liye
• Cardiac monitoring (ECG) — tachyarrhythmias, ST-T changes ke liye

✅ STEP 5 — Monitoring

• Vitals har 15-30 min regularly
• Neurological status monitor karo
• Urine output dekho (renal toxicity possible)
• Liver function bhi check karo severe cases mein

✅ STEP 6 — Disposition (Discharge ya Admit?)

Discharge instructions:

• Wound care sikhaao
• Return karo agar: wheezing, dyspnea, hives, dizziness, dysphagia ho
• Serum sickness ke baare mein batao (antivenom ke baad): fever, joint pain, rash — 7-10 din mein ho sakta hai

⚠️ Special Notes (India ke context mein):

• Buthotus tamulus (Red scorpion) — India mein sabse dangerous, severe cardiovascular complications deta hai
• Prazosin is the drug of choice cardiovascular management mein Indian scorpion envenomation mein
• Bacchon mein symptoms zyada severe aur jaldi aate hain — aggressive management karo
