  • Loosen tight clothing - collar, bra strap, anything tight around chest or neck
  • Do NOT give her food or water
  • Keep her calm - talk to her gently, hold her hand
  • Do not leave her alone

🚨 STEP 4 - If She Becomes Unconscious

  • Check if she is breathing - watch her chest
  • If NOT breathing - start CPR:
    • Place both hands on center of her chest
    • Push down hard and fast - 30 times
    • Then give 2 breaths into her mouth (if you can)

Stay calm. You are doing the right thing by asking. Call 112 NOW.

Actually she comes from hospital...I have remember one of his medicine sustac..she takes that

Sustac (Glyceryl Trinitrate / Nitrate tablet) - For Chest Pain/Angina

Sustac is a heart medicine for chest pain (angina). You can help her take it.

āœ… DO THIS NOW:

1. Find her medicine bag/purse
  • Look for the Sustac tablets - small bottle or strip
2. Give her 1 tablet
  • Place 1 tablet under her tongue (she must not swallow it - let it melt there)
  • OR if it is a spray - 1 spray under the tongue
3. Let her sit up
  • Do NOT lay her flat if she is conscious
  • Sitting position is better for heart patients
4. Wait 5 minutes
  • If pain does not go away after 5 minutes - give 1 more tablet under tongue
  • Maximum 3 tablets total, one every 5 minutes

āš ļø IMPORTANT - Do NOT give if:

  • She is unconscious
  • Her blood pressure is very low (feeling very faint, pale, weak)

🚨 STILL CALL 112 or 108

Even if she feels better after the tablet - she must go to hospital.
Sustac only relieves the pain temporarily - it does not fix the problem.

Is she conscious and able to speak? Does she have her medicine with her? Tell me and I will guide you further.

She is conscious..and after taking sustac she is better...we have opened window....she is lyingin her son's lap...what should we do if again she agains critical

Good News - She is Stable Now šŸ‘

The Sustac worked and she is conscious. You did well. Keep watching her carefully.

āœ… RIGHT NOW - Keep Doing This:

  • Windows open - fresh air is good, keep it open āœ“
  • Let her rest in her son's lap - comfortable position is fine āœ“
  • Talk to her softly - keep her calm, no stress
  • Do not give food or water yet
  • Watch her face - if it turns pale, blue lips, or she sweats heavily - act fast

🚨 IF SHE BECOMES CRITICAL AGAIN:

Step 1 - Give Sustac again
  • 1 tablet under the tongue immediately
  • She can take up to 3 tablets total (one every 5 minutes)
  • Count how many she already took - do not exceed 3 total
Step 2 - Call 112 or 108 immediately
  • Do not wait this time - call while giving tablet
Step 3 - If she becomes UNCONSCIOUS
  • Lay her flat on the floor of the bus
  • Check if she is breathing (watch chest rise)
  • If NOT breathing - start chest compressions:
    • Both hands, center of chest
    • Push down hard 30 times fast
    • 2 breaths into mouth
    • Repeat until help comes
Step 4 - Stop the bus
  • Tell driver to stop at nearest hospital or clinic immediately

šŸ“ Most Important Thing Now:

Her son should take her to hospital today - even though she feels better now. Sustac only relieves symptoms. The doctor must check her.

Watch for these WARNING SIGNS:

SignWhat it means
Chest pain returnsGive Sustac, call 112
Sweating heavilyGetting worse - call 112
Blue/pale lipsVery serious - call 112 now
Stops respondingUnconscious - start CPR
Breathing becomes difficultCall 112 immediately

You handled this very well. Stay with her and keep her calm. Is her son with her? Make sure he takes her to a hospital as soon as you reach the destination.

Which position is better for her... remember we are travelling in bus

Best Position in a Bus - Heart Patient

āœ… BEST POSITION - Semi-Sitting (Half Lying)

        HEAD & BACK raised up
             /
            /
/ ← Her back at an angle
          / (like a reclining chair)
_________/
   LEGS flat on the seat
How to do it in bus seat:
  • Her back should be slightly raised - not fully flat, not fully sitting
  • Use a bag, rolled jacket, or cloth behind her back as support
  • Her legs should be flat on the seat (not hanging down)
  • Head resting on her son's shoulder or a soft support

āŒ DO NOT DO:

  • āŒ Do NOT make her sit fully upright - puts strain on heart
  • āŒ Do NOT lay her completely flat - makes breathing harder
  • āŒ Do NOT let her legs hang down from the seat

šŸ’” Simple Way to Remember:

"Half sitting, like watching TV in bed"
  • Comfortable angle, breathing easy, legs up on seat

If She Feels Dizzy or Faint:

  • Then lay her more flat and raise her legs slightly
  • This helps blood flow to the brain

Is she comfortable right now? Is her breathing normal?

If she again critical should I give her sustac ..give orally with water or put it beneath tongue

Always Under the Tongue - NEVER Swallow with Water

āœ… CORRECT WAY:

šŸ‘… UNDER THE TONGUE

  • Place the tablet under her tongue
  • She must let it melt/dissolve there
  • Do NOT swallow it
  • Do NOT drink water with it
  • Do NOT chew it
  • Just keep it under tongue and let it dissolve slowly

Why under the tongue?

Because under the tongue has many blood vessels very close to skin - the medicine goes directly into blood within 1-2 minutes and reaches the heart very fast.
If she swallows it with water - it goes to stomach first - takes too long - heart does not get medicine fast enough.

Simple steps if she gets critical again:

StepAction
1Ask her to open her mouth
2Lift her tongue gently
3Place 1 Sustac tablet underneath
4Tell her - "Do not swallow, let it melt"
5Wait 5 minutes
6If no relief - give 1 more (max 3 total)
7Call 112 at the same time

āš ļø Remember:

  • Maximum 3 tablets - one every 5 minutes
  • Always call 112 when giving the tablet - do not wait to see if it works first

You are doing a great job helping her. How is she feeling now?

If she again critical should I give her sustac ..give orally with water or put it beneath tongue

Always UNDER THE TONGUE šŸ‘…

āŒ NOT with water

āœ… UNDER the tongue

Just 3 things to remember:

1. Place tablet UNDER her tongue

2. Let it MELT - do not swallow

3. NO water needed

Why?

Under tongue = medicine reaches heart in 1-2 minutes ⚔
Swallow with water = goes to stomach = takes too long = dangerous delay āŒ

Quick Reminder Table:

āŒ WRONGāœ… RIGHT
Swallow with waterUnder the tongue
Chew the tabletLet it dissolve slowly
Give when unconsciousOnly when she is awake

Same answer as before - under the tongue is the only correct way every time. šŸ‘
How is she doing now? Has she reached her destination yet?

She is feeling nausea what to do..actually ishe is patient of AWMI

🚨 AWMI + Nausea = SERIOUS EMERGENCY - Call 112 NOW

AWMI = Acute Wall Myocardial Infarction = Active Heart Attack
Nausea is a classic sign the heart attack is getting worse.

šŸ“ž CALL 112 IMMEDIATELY - Do not wait

Do this RIGHT NOW:

1. Call 112 or 108 - Tell them:
  • "Woman having heart attack - AWMI patient"
  • "She has nausea and chest problem"
  • Tell your exact location / bus route / nearest landmark
2. Tell the bus driver to stop at nearest hospital - do not wait for destination
3. Keep her in semi-sitting position - do not lay flat
4. Keep window open - fresh air
5. Do NOT give food, water, or anything by mouth right now
  • Nausea means she may vomit
  • Risk of choking
6. If she vomits:
  • Turn her gently to the left side
  • Clean her mouth
  • Keep her airway clear
7. Keep her calm - no stress, no panic around her

āš ļø AWMI is very dangerous:

Symptom she may haveWhat it means
Nausea / vomitingHeart attack worsening
SweatingDangerous sign
Chest pain returningGive Sustac under tongue
Pale or blue lipsCritical - CPR may be needed
Loses consciousnessStart CPR immediately

šŸ„ She MUST reach hospital NOW

AWMI patients cannot wait. Every minute damages the heart more.
Please stop the bus and get her to nearest hospital immediately.
Is there a hospital or clinic nearby on your route?

Is she suffering from gas..in iletryae language

No - This is NOT Gas. This is HEART ATTACK.

Many people think heart attack = gas problem. This is WRONG and DANGEROUS.

Simple difference:

Gas ProblemHeart Attack (AWMI)
Healthy personShe already has heart disease
Comes after eatingCan come anytime
Burping gives reliefBurping does NOT help
No sweatingSweating is common
Gets better on its ownGets WORSE without medicine
No nausea with chest painNausea + chest pain together

She has ALL signs of heart attack:

āœ… She is already a known heart patient
āœ… She was coming from hospital
āœ… She had chest pain - Sustac gave relief (Sustac only works for heart - not gas)
āœ… Now she has nausea - classic heart attack sign

Most important thing to understand:

šŸ«€ Sustac tablet worked = It IS a heart problem

Gas medicine would not have helped her. Sustac only works for heart. So her problem is definitely the heart - not gas.

🚨 Please stop the bus now and take her to hospital

Every minute is very important for AWMI patient.
Has the bus stopped? Is help coming?

Should I give sustac to her again

Only If Chest Pain Returns

Ask her RIGHT NOW:

"Do you have chest pain?"

YES - chest pain:

āœ… Give 1 Sustac under tongue NOW

  • Count how many she has taken already
  • Maximum 3 tablets total

NO - only nausea, no chest pain:

āŒ Do NOT give Sustac now

  • Sustac can lower blood pressure
  • If she is already feeling weak/nauseous - Sustac without chest pain can make her more faint and dizzy

āš ļø She has nausea right now - so:

  • Do NOT give anything by mouth - she may vomit
  • If she feels like vomiting - turn her to LEFT side immediately
  • Keep her calm
  • Fresh air - window open

🚨 Most important right now:

STOP THE BUS - GET TO HOSPITAL

Sustac is a temporary relief only.
She is an AWMI patient with nausea - this is an emergency.
No tablet can replace hospital treatment right now.

Does she have chest pain right now - YES or NO?
