2025 American Heart Association high blood pressure categories 140 90 diagnosis home blood pressure measurement

A resting reading of 140/87 mmHg is not an emergency by itself, but the systolic value is high. Under current AHA/ACC categories, it falls in stage 2 hypertension because the top number is 140 or higher. A single reading does not diagnose hypertension. The AHA home-monitoring guidance recommends looking at repeated readings over time.
What to do next
  1. Recheck correctly: Sit quietly for 5 minutes, back supported, feet flat, arm supported at heart level, no caffeine/nicotine/exercise for 30 minutes, and use a correctly sized upper-arm cuff.
  2. Measure at home for 7 days: Take two readings, 1 minute apart, in the morning and evening. Record them and calculate the average, ideally excluding the first day. Alternatively, arrange 24-hour ambulatory BP monitoring.
  3. Book a primary-care appointment within a few weeks, sooner if repeated home readings are around or above 140 systolic. At age 25, a clinician should confirm the pattern and review possible contributors or secondary causes, including family history, kidney disease, sleep apnea, thyroid problems, medications or substances, alcohol, stimulants, decongestants, NSAID painkillers, and recreational drugs.
  4. Start lifestyle measures now: reduce salt and packaged foods, exercise at least 150 minutes/week, maintain or work toward a healthy weight, limit alcohol, avoid smoking/vaping nicotine and stimulant drugs, prioritize sleep, and follow a DASH-style diet.
  5. Medication: Do not self-start medication. If the average confirmed BP is at least 140/90, current guidance generally recommends antihypertensive medication alongside lifestyle changes, individualized by a clinician. The 2025 AHA/ACC guidance recommends drug treatment for average BP at or above 140/90; stage 2 hypertension often needs prompt treatment planning.
Seek urgent care now if BP reaches over 180/120, especially with chest pain, shortness of breath, severe headache, weakness/numbness, confusion, vision changes, or trouble speaking.
A recent measurement-method review supports confirming elevated office readings using standardized home or ambulatory monitoring (PMID: 40825202).

CDC smoking nicotine raises blood pressure immediately tobacco hypertension

Yes. Tobacco, especially nicotine, can raise blood pressure temporarily by stimulating the sympathetic nervous system, increasing heart rate and constricting blood vessels. Cigarettes, bidis, chewing tobacco, nicotine pouches, and many vapes can all do this.
Smoking also damages blood vessels and raises the long-term risk of heart attack, stroke, and kidney disease. The CDC notes that nicotine raises BP and tobacco use increases high-BP risk.
For an accurate BP check, avoid smoking, vaping, or chewing tobacco for at least 30 minutes before measuring. If you use tobacco and your readings remain near 140 systolic, stopping tobacco is an important part of treatment, along with proper repeat BP monitoring and a clinician visit.
Nicotine-replacement products can also cause a small temporary BP rise, but are generally much safer than smoking and can be used as part of a clinician-guided quit plan.
