Diagnosis: Miliaria Rubra (Prickly Heat / Heat Rash)

The image shows a diffuse, fine papular eruption over the back on darkly pigmented skin, consistent with miliaria rubra - the most common form of heat rash. Compare with the textbook image of miliaria rubra above.

Pathophysiology

Occlusion of eccrine sweat ducts (partly due to Staphylococcus epidermidis extracellular polysaccharide) prevents sweat from reaching the skin surface. When the duct ruptures at the prickle cell layer (stratum spinosum), sweat leaks into surrounding tissue causing spongiosis and the characteristic inflammatory papules.

Clinical Features (Miliaria Rubra)

  • Discrete, extremely pruritic, erythematous papulovesicles (1-3 mm)
  • Sensation of prickling, burning, or tingling ("prickly heat")
  • Lesions may become confluent on a bed of erythema
  • Common sites: trunk (as seen here), antecubital/popliteal fossae, inframammary folds, abdomen, inguinal regions
  • Hot, humid climates or occlusive clothing are classic triggers

Treatment

Per Andrews' Diseases of the Skin, the key steps are:

Additional tips:
  • Wear loose, lightweight, breathable (cotton) clothing
  • Avoid occlusive creams or heavy moisturizers that worsen pore blockage
  • Topical calamine lotion or low-potency hydrocortisone 1% cream can help relieve itch
  • If secondary bacterial infection develops (pustular miliaria), a topical or short course of oral antibiotic targeting S. epidermidis may be needed
Prognosis: Resolves spontaneously once heat exposure is removed. Note that postmiliarial hypohidrosis (reduced sweating) can persist for up to 3 weeks after resolution, so avoid strenuous activity in heat during recovery.

Practical Advice (Pakistan summer context)

  • Air-conditioning or ceiling fan directly on skin is the single most effective treatment
  • Wear loose cotton shalwar kameez - avoid synthetic fabrics
  • Avoid heavy oils (mustard oil, coconut oil) on the back - these block pores and worsen miliaria
  • Bathe 2x/day with mild soap and cool water
