| Fracture (acute) | If there was a recent fall or blow, an undisplaced or displaced metacarpal fracture is possible - but the visible muscle wasting suggests some chronicity |
| Malunited old fracture | A previously broken bone that healed in a slightly abnormal position - could explain the bony prominence |
| Carpal boss | A bony osteophyte at the base of the metacarpals - a common benign finding |
| Ganglion cyst | A soft tissue lump - typically feels slightly compressible, not bony |
| Joint subluxation/dislocation | Partial or complete dislocation at a carpometacarpal joint |
