Hanau's Quint

Hanau's Quint refers to the five factors that govern balanced occlusion in complete dentures, described by Rudolph Hanau (originally proposed as nine factors, later condensed to five). Achieving balance among these five is the goal when setting artificial teeth so that simultaneous contact occurs on both sides during excursive movements.

The Five Factors

2Incisal GuidanceThe angle formed by the incisal edges of the anterior teeth and the horizontal plane during protrusion. A steep incisal guidance disrupts posterior balance and must be compensated by other factors.
3Occlusal Plane InclinationThe orientation of the occlusal plane relative to the ala-tragus line or the horizontal. Tilting the plane affects the geometric balance between the front and back teeth.
4Compensating CurveThe anteroposterior (sagittal) and lateral curvature built into the occlusal surfaces of the posterior teeth (analogous to the curve of Spee/Wilson). Increasing the curve compensates for steep condylar or incisal guidance.
5Cuspal Inclination (Cusp Height)The angle of the cusp inclines relative to the occlusal plane. Higher, steeper cusps provide better balance but reduce stability on a resilient denture-bearing ridge.

Theilman's Formula

The inter-relationship of all five factors is expressed as Theilman's formula:
C (Condylar Inclination) = (Occlusal Plane × Incisal Guidance × Cuspal Inclination) / Compensating Curve
When all five factors are in proportion, balanced occlusion is maintained during protrusive and lateral excursions - meaning posterior teeth remain in contact bilaterally throughout all mandibular movements.

Key Clinical Points

  • Condylar inclination is the only fixed factor - it is dictated by the patient's anatomy and recorded via a facebow/articulator. All other four factors are adjustable by the prosthodontist.
  • Increasing any one factor requires compensating changes in the others to maintain balance.
  • Balanced occlusion in complete dentures is specifically aimed at stability - preventing denture rotation/tipping during chewing - unlike in natural dentition where group function or canine guidance is preferred.
  • Flat (0°) cusps (monoplane occlusion) can achieve balance by eliminating cusp inclines, relying entirely on the occlusal plane and compensating curve.
