Here is a thorough explanation of AMTSL:
AMTSL - Active Management of the Third Stage of Labour
What is the Third Stage of Labour?
The third stage of labour spans from the delivery of the baby to the delivery of the placenta and membranes. This stage carries the highest risk of postpartum haemorrhage (PPH), which is the leading cause of maternal mortality worldwide (responsible for roughly 25% of all maternal deaths).
What is AMTSL?
AMTSL is a package of proactive clinical interventions performed routinely after every birth to prevent excessive blood loss and reduce PPH. It is universally recommended by the WHO, FIGO, ICM, ACOG, and RCOG.
It is preferred over expectant (physiologic) management, where the clinician simply waits for the placenta to deliver on its own without interventions.
The Three Core Components of AMTSL
| Step | Intervention | Detail |
|---|
| 1 | Uterotonic administration | Oxytocin (Pitocin) 10 IU IM is the drug of choice. Given with or shortly after delivery of the anterior shoulder (before placental separation). Alternatives: misoprostol, ergometrine, carbetocin. |
| 2 | Controlled Cord Traction (CCT) | Gentle, sustained traction on the umbilical cord while applying counter-pressure (Brandt-Andrews manoeuvre) on the lower uterine segment to guide the placenta out. |
| 3 | Uterine massage | Massage of the uterine fundus after placental delivery to stimulate sustained uterine contraction and reduce bleeding. |
Why is AMTSL Important?
- Decreases the duration of the third stage
- Reduces blood loss by approximately two-thirds
- Reduces the need for blood transfusion
- Reduces the need for additional uterotonic drugs
- Significantly cuts maternal mortality from PPH

AMTSL vs. Expectant Management
| Feature | AMTSL | Expectant Management |
|---|
| Uterotonic | Given routinely | Given only if bleeding occurs |
| Cord clamping | Early (after uterotonic given) | Delayed, spontaneous |
| Placenta delivery | Assisted by cord traction | Spontaneous expulsion |
| Blood loss | Significantly less | Higher risk |
| Recommended for | All births | Selected low-risk cases |
In summary, AMTSL is the standard of care at every birth to prevent the number one cause of maternal death - postpartum haemorrhage.
