For an adult with CrCl 40 mL/min, use the usual loading regimen, then from day 4 onward reduce the maintenance exposure by 50%:

Example: if the intended maintenance regimen is 400 mg once daily, use 400 mg every 48 h or 200 mg once daily from day 4.

Check teicoplanin trough concentrations where available, especially in severe infection, obesity, unstable renal function, or prolonged therapy. The product information targets at least 10 mg/L by HPLC for many infections, with higher targets commonly used for deep-seated infections according to local protocol. This renal adjustment is supported by the Targocid SmPC.

Do not reduce the initial loading doses solely for CrCl 40 mL/min. Use local antimicrobial guidance and infection-specific dosing.
