Gabapentin Dosing in Adults

Gabapentin (Neurontin) dosing depends on the indication. It has non-linear (saturable) absorption - as the dose increases, the fraction absorbed decreases - which is why it's given in divided doses and titrated gradually.

1. Epilepsy (adjunctive therapy for focal/partial seizures)

  • Start: 300 mg on day 1, titrating up over the first few days (or per some regimens, starting at 900 mg/day in 3 divided doses)
  • Usual maintenance: 900-1800 mg/day in 3 divided doses
  • Maximum: up to 3600 mg/day if higher doses are needed for seizure control

  • Gabapentin is considered a first-line co-analgesic for neuropathic pain, particularly favored in elderly and medically ill patients because of its favorable safety profile and low drug-interaction potential

Renal Impairment - critical adjustment

Gabapentin is eliminated almost entirely unchanged by the kidneys and has a narrow therapeutic window in renal impairment. Dose must be reduced based on creatinine clearance in any patient with reduced renal function, and even more so in dialysis-dependent patients (dosed after dialysis only). Failure to adjust can cause accumulation and neurotoxic effects (sedation, myoclonus, encephalopathy).

Key practical points

  • Divide total daily dose into 3 doses/day for most indications (except pruritus, which is post-dialysis dosing).
  • Titrate slowly to minimize dizziness, somnolence, and unsteadiness - the most common adverse effects.
  • Always individualize based on renal function, indication, and tolerability rather than jumping straight to high doses.
Note on evidence currency: A 2025 systematic review flagged potential cardiovascular safety signals (VTE, arrhythmia risk) with gabapentinoids (PMID: 40536375), and gabapentin misuse/dependence has drawn increasing clinical attention - worth considering in patients with cardiovascular risk factors or substance use history, though this doesn't change the standard dosing above.
