Congestive Heart Failure (CHF) is a broad topic. Here's a quick map of what it covers, then let me know where you want to go deeper.

Overview

CHF is a clinical syndrome in which the heart cannot pump enough blood to meet the body's metabolic demands, or can only do so at elevated filling pressures. It results from structural or functional cardiac abnormalities (systolic dysfunction, diastolic dysfunction, or both) and presents with dyspnea, fatigue, and fluid retention (edema, pulmonary congestion). Major covered sources include Braunwald's Heart Disease, Textbook of Family Medicine, Brenner and Rector's The Kidney (cardiorenal aspects), and Murray & Nadel's Respiratory Medicine (cardiopulmonary overlap).

Topic breakdown

  1. Pathophysiology & classification - systolic (HFrEF) vs diastolic (HFpEF), Frank-Starling mechanism, neurohormonal activation (RAAS, sympathetic), remodeling
  2. Etiology & risk factors - ischemic heart disease, hypertension, valvular disease, cardiomyopathies, arrhythmias
  3. Clinical presentation - symptoms/signs, NYHA functional classification, ACC/AHA staging
  4. Diagnosis - history/exam, BNP/NT-proBNP, echocardiography, chest X-ray, ECG
  5. Pharmacologic management - ACE inhibitors/ARBs/ARNI, beta-blockers, mineralocorticoid antagonists, SGLT2 inhibitors, diuretics, digoxin
  6. Device & advanced therapy - CRT, ICD, mechanical support (LVAD), transplant
  7. Acute decompensated heart failure - presentation and emergency management
  8. Special considerations - cardiorenal syndrome, HF with preserved EF management, comorbidities (CKD, COPD)
